目的基于文献研究，探讨年龄相关性黄斑变性（age-related macular degeneration，AMD）患者的舌象分布特征，总结其中医致病因素及辨证规律。 方法以“黄斑变性”“中医”为关键词，检索中国知网（CNKI）自建库至2021年12月31日的相关文献，筛选出其中有明确舌象记录的文献，提取舌色、舌形、舌苔等信息，采用频数统计法进行分析。 结果共纳入有舌象记录的文献27篇，涉及AMD患者159例。舌色方面，以舌暗红（72例，47.06%）、舌淡白（36例，23.53%）、舌红（33例，21.57%）为主，暗舌类占比较高；舌形方面，胖大舌（4例，40.00%）、齿痕舌（3例，30.00%）、瘀斑舌（3例，30.00%）均有出现；舌苔方面，以薄苔（90例，58.06%）、白苔（44例，28.39%）、黄苔（28例，18.06%）最为常见。 结论AMD患者舌象以暗红、淡白、红舌为主，兼见胖大、齿痕、瘀斑等舌形变化，舌苔多呈薄、白、黄苔。提示AMD患者体质多以属阴虚火旺或脾肾阳虚为本，痰浊、湿热、瘀血为标。临床疗效较好的患者病程较短、病邪较浅，提示AMD患者应尽早接受中医干预治疗。
Abstract:
ObjectiveBased on literature research, to investigate the tongue manifestation distribution characteristics of patients with age-related macular degeneration (AMD), and to summarize the associated pathogenic factors and pattern differentiation principles in traditional Chinese medicine. MethodsRelevant literature was retrieved from the CNKI database from database establishment to December 31, 2021 using the keywords "macular degeneration" and "traditional Chinese medicine". Publications with explicit tongue manifestation records were screened, and data on tongue color, tongue shape, and tongue coating were extracted and analyzed using frequency statistics. ResultsA total of 27 publications with tongue manifestation records were included, involving 159 patients with AMD. Regarding tongue color, dark red tongue (72 cases, 47.06%), pale white tongue (36 cases, 23.53%), and red tongue (33 cases, 21.57%) were predominant, with dark-toned tongues accounting for a relatively high proportion. Regarding tongue shape, enlarged tongue (4 cases, 40.00%), scalloped tongue (3 cases, 30.00%), and ecchymotic tongue (3 cases, 30.00%) all appeared. Regarding tongue coating, thin coating (90 cases, 58.06%), white coating (44 cases, 28.39%), and yellow coating (28 cases, 18.06%) were the most common. ConclusionThe tongue manifestations of AMD patients are predominantly dark red, pale white, and red tongue, accompanied by tongue shape variations such as enlarged tongue, scalloped tongue, and ecchymotic tongue. The tongue coating is mostly thin, white, or yellow in appearance. These findings suggest that the constitution of AMD patients is primarily characterized by Yin deficiency with fire hyperactivity or spleen-kidney Yang deficiency as the root cause, with phlegm-turbidity, damp-heat, and blood stasis as the superficial manifestations. Patients who show good clinical response tend to have shorter disease duration and more superficial pathogenic involvement, indicating that AMD patients should receive early TCM intervention.
