摘要:
目的观察滋肾清胃解毒方治疗牙周病的临床疗效及对相关生化指标的影响。 方法将80例牙周病患者随机分为对照组和治疗组，每组40例。对照组予常规基础治疗，治疗组在对照组基础上，辅以滋肾清胃解毒方治疗，两组治疗均以14天为1个疗程，治疗3个疗程。比较两组患者治疗前后牙周炎检测指标、外周血炎症因子水平［白细胞介素6（interleukin-6，IL-6）、IL-8、IL-1β、肿瘤坏死因子α（tumor necrosis factor α，TNF-α）］、氧化应激指标［超氧化物歧化酶（superoxide dismutase，SOD）、谷胱甘肽过氧化物酶（glutathione peroxidase，GSH-Px）、丙二醛（malondialdehyde，MDA）］及临床疗效和不良反应。 结果两组患者治疗后牙周炎各项检测指标均较治疗前下降（P＜0.05），治疗组降低幅度较对照组显著（P＜0.05）；两组患者治疗后外周血炎症因子水平IL-6、IL-8、IL-1β、TNF-α均较治疗前降低（P＜0.05），治疗组患者IL-1β、TNF-α均低于对照组（P＜0.05），IL-6、IL-8组间比较差异无统计学意义（P＞0.05）；两组患者治疗后血清GSH-Px、SOD均较治疗前升高，血清MDA降低（P＜0.05），且治疗组指标改善优于对照组（P＜0.05）；治疗组临床总有效率［95％（38/40）］高于对照组［70％（28/40）］（P＜0.05）；两组患者不良反应率比较差异无统计学意义（P＞0.05）。 结论在常规牙周炎治疗基础上，采用滋肾清胃解毒方治疗肾阴虚胃火盛型牙周炎疗效确切，其具有减轻机体炎症程度及氧化应激状态的作用，还可改善牙周组织病变程度，且用药安全。
Abstract:
ObjectiveTo observe the clinical efficacy of Zishen Qingwei Jiedu Formula in the treatment of periodontal disease and its effects on relevant biochemical indicators. MethodsA total of 80 patients with periodontal disease were randomly divided into a control group and a treatment group, with 40 cases in each group. The control group received routine basic therapy, while the treatment group received Zishen Qingwei Jiedu Formula in addition to the same basic therapy. Both groups were treated for three courses, with 14 days constituting one course. The following parameters were compared between the two groups before and after treatment: periodontal examination indices, serum levels of inflammatory cytokines [interleukin-6 (IL-6), IL-8, IL-1β, and tumor necrosis factor-α (TNF-α)], oxidative stress markers [superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and malondialdehyde (MDA)], as well as clinical efficacy and adverse reactions. ResultsAfter treatment, all periodontal examination indices decreased significantly in both groups compared with baseline values (P<0.05), and the magnitude of reduction was significantly greater in the treatment group than in the control group (P<0.05). Serum levels of inflammatory cytokines, including IL-6, IL-8, IL-1β, and TNF-α, were significantly reduced after treatment in both groups compared with pretreatment levels (P<0.05). The post-treatment levels of IL-1β and TNF-α in the treatment group were significantly lower than those in the control group (P<0.05), whereas there were no statistically significant differences between the two groups in IL-6 and IL-8 levels (P>0.05). Serum GSH-Px and SOD levels were significantly elevated after treatment in both groups, while serum MDA levels were significantly decreased (P<0.05), with significantly greater improvements observed in the treatment group compared with the control group (P<0.05). The total clinical effective rate in the treatment group [95% (38/40)] was significantly higher than that in the control group [70% (28/40)] (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). ConclusionOn the basis of conventional periodontal therapy, the adjunctive use of Zishen Qingwei Jiedu Formula demonstrates definitive therapeutic efficacy in treating periodontal disease of the pattern characterized by Kidney-Yin deficiency and Stomach-Fire excess. It effectively alleviates systemic inflammatory responses and oxidative stress, ameliorates the severity of periodontal tissue lesions, and exhibits a favorable safety profile.
