摘要:
目的探讨温针灸八髎穴联合温肾地黄汤与氯米芬对肾阳虚型多囊卵巢综合征（polycystic ovary syndrome，PCOS）排卵障碍性不孕症患者的干预效果。 方法选取90例肾阳虚型PCOS排卵障碍性不孕症患者，随机分为对照组与治疗组各45例。对照组予氯米芬治疗，治疗组在对照组基础上加用温针灸八髎穴与温肾地黄汤治疗。比较两组患者临床疗效、妊娠率、中医证候积分、卵巢功能［子宫内膜厚度、卵巢体积、卵泡数目］、血清性激素［孕酮（progesterone，P）、雌二醇（estradiol，E2）、促黄体生成素、促卵泡激素水平及不良反应发生率。结果治疗后，总有效率治疗组［86.67%（39/45）］高于对照组［68.89%（31/45）］（P＜0.05）；妊娠率治疗组［40.00%（18/45）］高于对照组［33.33%（15/45）］（P＜0.05）；治疗组子宫内膜厚度及血清P、E2、FSH水平高于对照组（P＜0.05），卵巢大小、卵泡数量及血清LH水平低于对照组（P＜0.05）。 结论温针灸八髎穴联合温肾地黄汤结合氯米芬治疗肾阳虚型PCOS排卵障碍性不孕症，可提高临床疗效，减轻患者临床症状，改善卵巢功能，调节性激素水平，安全性良好。
Abstract:
ObjectiveTo investigate the interventional effects of warm needle acupuncture at eight Liao points combined with Wenshen Dihuang Decoction and clomifene on patients with ovulatory disorder infertility due to polycystic ovary syndrome (PCOS) of kidney Yang deficiency syndrome. MethodsA total of 90 patients with ovulatory disorder infertility due to PCOS of kidney Yang deficiency syndrome were enrolled and randomly divided into a control group and a treatment group, with 45 cases in each group. The control group received clomifene alone. The treatment group received warm needle acupuncture at eight Liao points and Wenshen Dihuang Decoction in addition to the clomifene therapy. The clinical efficacy, pregnancy rate, TCM syndrome scores, ovarian function parameters (endometrial thickness, ovarian volume, and follicle count), serum sex hormone levels inlcuding progesterone (P), estradiol (E2), luteinizing hormone (LH), and follicle-stimulating hormone (FSH)], and the incidence of adverse reactions were compared between the two groups. ResultsAfter treatment, the total effective rate in the treatment group [86.67% (39/45)] was significantly higher than that in the control group [68.89% (31/45)] (P<0.05). The pregnancy rate in the treatment group [40.00% (18/45)] was also significantly higher than that in the control group [33.33% (15/45)] (P<0.05). Furthermore, the treatment group showed significantly increased endometrial thickness and serum levels of P, E2, and FSH compared with the control group (P<0.05), while ovarian volume, follicle count, and serum LH level were significantly decreased in the treatment group compared with the control group (P<0.05). ConclusionWarm needle acupuncture at eight Liao points combined with Wenshen Dihuang Decoction and clomifene can effectively improve clinical efficacy, alleviate clinical symptoms, enhance ovarian function, and regulate serum sex hormone levels in patients with kidney Yang deficiency-type PCOS-related ovulatory disorder infertility, with a favorable safety profile.
