摘要:
目的观察芍药四君健脾加减方联合内镜下西药喷洒治疗溃疡性结肠炎（ulcerative colitis，UC）的临床效果。 方法将280例UC患者按治疗方案分为甲、乙、丙三组。甲组90例采用内镜下西药喷洒治疗，乙组90例采用芍药四君健脾加减方治疗，丙组100例采用芍药四君健脾加减方联合内镜下西药喷洒治疗。比较三组患者治疗后中医症状评分、炎性因子水平、UC内镜下严重程度指数（ulcerative colitis endoscopic index of severity，UCEIS）评分及肠黏膜屏障功能指标。 结果治疗1个月及随访3个月后，甲、乙两组患者内镜下UC活动度比较差异无统计学意义（P＞0.05），丙组患者内镜下UC活动度低于甲、乙两组（P＜0.05）；甲、乙两组患者大便稀溏、里急后重、黏液脓血便、乏力、纳差等中医症状评分比较差异无统计学意义（P＞0.05），丙组患者上述症状评分均低于甲、乙两组（P＜0.05）；甲、乙两组患者白细胞介素8（interleukin-8，IL-8）、IL-10、IL-13、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）及超敏C反应蛋白（high-sensitivity C-reactive protein，hs-CRP）水平比较差异无统计学意义（P＞0.05）；丙组患者IL-10、IL-13水平更高，IL-8、TNF-α、hs-CRP水平低于甲、乙组（P＜0.05）；甲、乙两组患者二胺氧化酶（diamine oxidase，DAO）、D-乳酸及内毒素水平比较差异无统计学意义（P＞0.05），丙组患者上述指标水平均低于甲、乙两组（均P＜0.05）。 结论芍药四君健脾加减方联合内镜下西药喷洒，可快速改善UC患者临床症状，抑制炎性因子作用，减轻肠黏膜损伤，有助于改善预后。
Abstract:
ObjectiveTo observe the clinical efficacy of the modified Shaoyao Sijun Jianpi Decoction combined with endoscopic Western drug spraying in the treatment of ulcerative colitis (UC). MethodsA total of 280 patients with UC were divided into three groups (Groups A, B, and C) according to the treatment regimen. The 90 patients in Group A received endoscopic Western drug spraying alone; the 90 patients in Group B received the modified Shaoyao Sijun Jianpi Decoction alone; and the 100 patients in Group C received the modified Shaoyao Sijun Jianpi Decoction combined with endoscopic Western drug spraying. The three groups were compared in terms of TCM symptom scores, levels of inflammatory factors, ulcerative colitis endoscopic index of severity (UCEIS) scores, and intestinal mucosal barrier function indicators after treatment. ResultsNo statistically significant difference in endoscopic UC activity was found between Group A and Group B after one month of treatment and three months of follow-up (P>0.05), Group C was lower than Group A and Group B in endoscopic UC activity (P<0.05). There was no statistically significant difference between Group A and Group B in the TCM symptom scores, including loose stools, tenesmus, mucopurulent bloody stools, fatigue, and poor appetite (P>0.05), the scores of the above symptoms in Group C were all lower than those in Groups A and B (P<0.05); There were no statistically significant differences in the levels of interleukin-8 (IL-8), IL-10, IL-13, tumor necrosis factor-α (TNF-α), and high-sensitivity C-reactive protein (hs-CRP) between Groups A and B (P>0.05). Patients in Group C had higher levels of IL-10 and IL-13, and lower levels of IL-8, TNF-α, and hs-CRP compared with Groups A and B (P<0.05). There were no statistically significant differences in the levels of diamine oxidase (DAO), D-lactic acid, and endotoxin between Groups A and B (P>0.05), whereas the levels of the above indicators in Group C were all lower than those in Groups A and B (all P<0.05). ConclusionThe combination of the modified Shaoyao Sijun Jianpi Decoction and endoscopic Western medicine spraying can promptly ameliorate the clinical manifestations of UC patients, exert anti-inflammatory effects by suppressing inflammatory cytokines, alleviate intestinal mucosal damage, and facilitate favorable prognostic outcomes.
