目的：通过分析《医学穷源集》中的咳嗽病案，挖掘王肯堂运气学术思想的临床应用经验，借此拓宽运气理论在现代临床中的应用范围。方法：通过关键词“咳” “嗽”检索《医学穷源集》并收集相关医案，利用Excel 2010软件建立医案数据库，对数据进行整理、统计，分析咳嗽医案的运气特征以及王氏的用药经验。结果：在112例运气医案中，共收集到13例咳嗽医案。从年份运气特点看，司天主要集中在少阳相火，在泉主要集中在厥阴风木。季节上春季易多发。疾病分析时可从“中运-客运-客气-月建”思路入手，用药方面要注意根据运气的变化而预防病变。结论：王肯堂临床应用运气理论认识疾病的发生发展，可为五运六气的现代应用提供借鉴。
Abstract:
Objective: To dig into Wang Kentang′s experience of clinical application of academic thinking, in the wish of widening clinical application of five movements and six climates theory in modern times by analyzing the medical cases of cough in YiXue QiongYuan Ji. Methods: YiXue QiongYuan Ji was searched by taking “Cough” as the key word, and the related medical cases were collected, the database of medical cases was established by using office excel 2010 software, the five movements and six climates features of cough cases and Wang′s medication experience were analyzed. Results: Among 112 cases, 13 cases of cough were collected. Celestial manager Qi mainly gathered at ShaoYang ministerial fire, Qi in the earth accumulated at JueYin and wind-wood. Cough was liable to happen in the spring. The disease analysis could start with the thinking of “ZhongYun-KeYun-KeQi-YueJian” and the prevention and progression of the disease should be valued according to the changes of movement and climates when using the medicinal. Conclusion: Wang Kentang using movement-climate theory in clinic to investigate the onset and development of the disease could provide the reference for modern use of five movements and six climates.
