摘要:
目的:通过对感染后咳嗽现代中医文献资料中病案的回顾性研究,分析并探讨该病中医证素和处方用药的分布、组合规律。方法:以感染后咳嗽的现代中医文献医案为研究对象,建立病案证候数据库,参照“证候辨证素量表”的简化计量权值分别对各个证候权值进行计算,将每个病案的证素判定结果与处方用药录入中医传承辅助平台(版本2.5),对证素及处方用药规律进行分析。结果:102篇纳入的医案中,证素总计326条,证素类型共计25种,关联规则分析得到常见的证素组合25个(支持度10,置信度0.7),证素组合关联规则23个,处方中药物总计159种,常见的处方药物组合28个(支持度25,置信度0.8),处方药物组合关联规则17个,演化出3-4味药核心组合10个(相关度9,惩罚度2),演化出新方组合5个。结论:通过中医传承辅助平台的多角度分析,提示感染后咳嗽兼有外感咳嗽和内伤咳嗽的特性,在治疗上需要两者兼顾,为临床对本病的中医治疗与研究提供了有益的线索。
Abstract:
Objective: To analyze the syndrome elements of postinfectious cough, the distribution and the combination rules of the prescription by studying medical records in the modern TCM literature of postinfectious cough retrospectively. Methods: Medical records of postinfectious cough in modern TCM literature were collected as the objects of study to establish the database of syndromes of the medical records, the weight values of different syndromes were calculated respectively by consulting simplified measurement weight values in the scales of syndrome differentiation and syndrome elements, the judging results of syndrome elements and the herbs in the prescriptions of each medical record were typed into TCMISS (version 2.5), syndrome elements and the administration rules of the prescriptions were analyzed. Results: Among 102 medical records that included, there were 326 items of syndrome elements altogether, 25 kinds of syndrome elements, the analysis of association rules showed that there were 25 common syndrome elements combinations (support rating 10, confidence level 0.7), 23 syndrome elements association rules, there were 159 kinds of herbs altogether in the prescriptions, 28 kinds of common drug combinations in the prescription (support rating 25, confidence level 0.8), 17 drug combination association rules in the prescription, evolved into ten core drug combinations containing three to four kinds of herbs (support rating 9, punishment degree 2), developed into five new prescription combinations. Conclusion: It is suggested that postinfectious cough possesses the features of exogenous cough and cough due to internal damage by multi - angle analysis of TCMISS, and the treatment should be considered from both, which could provide useful clues to TCM therapy and study for the disease in clinic.
