摘要:
目的：观察肘内翻矫形器治疗伸直型儿童肱骨髁上骨折的中远期临床疗效和肘内翻发生率。方法：将86例患儿随机分为观察组和对照组，2组均给予闭合手法复位后，观察组(45例)给予肘内翻矫形器固定；对照组(41例)给予石膏固定。并对所有患者进行中期随访(治疗后12～24个月)及远期随访(治疗后24～36个月)，参照Flynn标准进行肘关节功能评定。结果：所有患者均获得随访：1)中期随访，观察组优良率95.55%：对照组优良率为80.48％，2组比较差异有统计学意义(P＜0.05)。观察组肘内翻发生率为2.22％，明显少于对照组12.19％(P＜0.05)。2)远期随访，观察组优良率97.77%：对照组优良率为85.36％，2组比较差异有统计学意义(P＜0.05)。观察组肘内翻发生率为2.22％，明显少于对照组14.63％，2组比较差异有统计学意义(P＜0.05)。结论：2种治疗方法比较以肘内翻矫形器外固定，中远期临床疗效显著，肘内翻发生率低，值得临床推广应用。
Abstract:
Objective：To observe medium-term and long-term effects of varus elbow orthosesin the treatment for straightening type supracondylar fracture humerus in children and the incidence of cubitus varus. Methods: A total of 86 patients were randomized into the observation group and the control group, after both groups were reset by closed manipulation, the observation group (45 cases) were fixed by varus elbow orthoses; while the control group (41 cases) by plaster. Elbow function was evaluated according to Flynn standard after all the patients were observed in the medium-term follow-ups (from 12 to 24 months after treating) and the long-term follow-ups (from 24 to 36 months after treating). Results: All the patients were followed: 1) the medium-term follow-ups, the excellent rate of the observation group was 95.55%, higher than 80.48% of the control group, and the difference showed statistical meaning (P＜0.05). The incidence of supracondylar fracture humerus of the observation group was 2.22%, obviously lower than 12.19% of the control group (P＜0.05). 2) the long-term follow-ups, the excellent rate of the observation group was 97.77%, higher than 85.36% of the control group, and the difference was statistically significant in the comparison between both groups(P＜0.05). The incidence of cubitus varus of the observation group was 2.22%, apparently lower than to 14.63% of the control group (P＜0.05).Conclusion: The comparison between both therapies demonstrates that medium-term and long-term effects are notable, and the incidence of cubitus varus is low, and it deserves clinical spread and application.
