摘要:
目的探讨自拟黄归汤联合来氟米特对糖尿病肾病（diabetic nephropathy，DN）患者血清色素上皮源因子（pigment epithelium-derived factor，PEDF）水平的影响。 方法将DN患者132例随机分为3组各44例，对照1组口服自拟黄归汤，对照2组口服来氟米特，研究组口服自拟黄归汤及来氟米特。观察患者疗效及不良反应发生率，同时观察患者糖化血红蛋白（glycosylated hemoglobin，HbAlc）、空腹血糖（fasting blood glucose，FPG）、餐后2h血糖（2-hour postprandial blood glucose，2hPG）、PEDF、血管内皮生长因子（vascular endothelial growth factor，VEGF）及白细胞介素6（interleukin-6，IL-6）、白细胞介素11（interleukin-11，IL-11）和肿瘤坏死因子α（tumor necrosis factor α，TNF-α）等炎性因子以及肾功能。 结果不良反应发生率研究组为6.82%，低于对照组1组的27.27%和对照2组的22.73%（P＜0.05）；治疗后研究组患者HbAlc、FPG、2 hPG及PEDF、VEGF和肾功能改善优于对照1、2组（P＜0.05）；研究组有效率和IL-6、IL-11、TNF-α炎性因子水平优于对照1、2组（P＜0.05）。 结论应用自拟黄归汤联合来氟米特治疗DN疗效理想，能降低患者血清PEDF水平和不良反应发生率。
Abstract:
ObjectiveTo explore the effects of selfmade Huanggui decoction and leflunomide on the levels of serum pigment epithelium-derived factor (PEDF) in patients with diabetic nephropathy (DN). MethodsAll 132 DN patients were randomized into three groups, 44 cases in each group, the control group No.1 took selfmade Huanggui decoction, the control group No.2 leflunomide, and the study group selfmade Huanggui decoction and leflunomide. To observe clinical effects, the incidence of adverse reaction, HbA1c, FPG, 2hPG, the levels of PEDF, VEGF, IL-6, IL-11, TNF-α and renal function. ResultsThe incidence of adverse reaction of the study group was 6.82%, lower than 27.27% of the control group No.1 and 22.73% of the control group No.2 (P<0.05); after treating, the improvements of renal function, the levels of HbA1c, FPG, 2hPG, PEDF and VEGF of the study group were superior to these of the other two groups (P<0.05); the effective rate and the levels of IL-6, IL-11 and TNF-α of the study group were better than these of the other two groups (P<0.05). ConclusionThe application of selfmade Huanggui decoction and leflunomide could obtain ideal clinical effects, and it could lower the levels of PEDF and the incidence of adverse reaction.
