摘要:
目的观察基于“养肝扶脾”理论的针刺结合颞三针治疗产后缺乳性偏头痛的临床疗效。 方法对32例产后缺乳性偏头痛患者采用“养肝扶脾”选穴针刺并配合颞三针治疗。选穴：膻中、少泽、肩井、乳根，颞三针，并随症加减。膻中、乳根采用叩刺法，余穴常规针刺，颞三针连接电针，每次留针30 min，每天1次，5天1个疗程，共治疗2个疗程，疗程间隔2天。观察治疗前后患者头痛视觉模拟量表评分（visual analog scale，VAS）和头痛积分变化情况及临床疗效；通过乳汁分泌评估缺乳情况，分析缺乳与头痛疗效的相关性。 结果首次治疗后、疗程结束后以及随访期患者VAS评分均低于治疗前（P＜0.05），且疗程结束后及随访期患者VAS评分均低于首次治疗时（P＜0.05）；随访时患者的头痛积分低于治疗前（P＜0.05）；头痛总有效率为90.6%，缺乳总有效率为87.5%，且头痛疗效与缺乳疗效有相关性（P＜0.05）。治疗过程中及随访期均未出现不良反应。 结论基于“养肝扶脾”理论针刺结合颞三针对产后缺乳性偏头痛患者头痛和缺乳均有疗效，二者疗效存在关联性，且针刺具有操作安全、起效迅速、持久镇痛的优势。
Abstract:
ObjectiveTo observe clinical effects of acupuncture based on “nourishing liver and invigorating spleen” and temporal three-needle in treating postpartum hypogalactia migraine. MethodsAll 32 patients adopted acupuncture of “nourishing liver and invigorating spleen” according to acupoint selection method and temporal three-needle. Selecting the acupoints: Danzhong (RN17), Shaoze (SI1), Jianjing (GB21), Rugen (ST18), temporal three-needle, modification according to symptoms. Tapping needling on Danzhong and Rugen, conventional acupuncture on the other acupoints, acusector on temporal three-needle, retaining needles for 30 minutes each time, once each day, five days were one course, for two courses altogether, the interval was two days. To observe VAS of headache, headache integrals and clinical effects before and after the treatment; lactation was assessed by milk secretion, to analyze the connections between hypogalactia and clinical effects of headache. ResultsVAS scores by the end of the course, after the first treatment and at the follow-up were lower than these before the treatment (P<0.05), and VAS scores by the end of the course and at the follow-up were lower than these after the first treatment (P<0.05); headache scores at the follow-up were lower than these before treatment (P<0.05); Total effective rate of headache was 90.6%, and that rate of hypogalactia was 87.5%, and clinical effects of headache presented the correlation with clinical effects of hypogalactia (P<0.05). No adverse reaction occurred during the therapeutic course and the follow-up period. ConclusionAcupuncture based on "nourishing liver and invigorating spleen" and temporal three-needle is effective in treating postpartum hypogalactia migraine, clinical effects of both diseases show the connections, and acupuncture has the advantages of safe operation, fast onset of action and long-lasting analgesia.
