摘要:
目的应用系统评价/Meta分析方法评价经皮穴位电刺激对老年髋关节置换术后认知功能的影响。 方法系统检索PubMed、Cochrane library、Web of science、Embase、CBM、CNKI、VIP及WanFang数据库中关于经皮穴位电刺激用于髋关节置换手术的研究，时间从建库至2020年10月7日。由2名研究人员背靠背同时筛选文献，提取数据，使用Cochrane风险偏倚评估工具评价纳入研究的质量，并采用RevMan 5.3软件进行Meta分析。 结果共纳入5项随机对照研究，共计344例受试患者，Meta分析结果显示：试验组术后1天简易智能量表（mini-mental state examination，MMSE）评分和术后3天MMSE评分的下降程度、术后认知功能障碍（postoperative cognitive dysfunction，POCD）的发生率、术后肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）和白细胞介素1β（interleukin-1β，IL-1β）的上升水平均低于对照组（P＜0.05）。两组患者术后白细胞介素6（interleukin-6，IL-6）的上升水平差异无统计学意义（P＞0.05）。 结论经皮穴位电刺激可降低老年髋关节置换术患者POCD的发生率，抑制术后早期炎性因子的释放。
Abstract:
ObjectiveTo assess the influence of transcutaneous electrical acupoint stimulation (TEAS) on cognitive function after hip arthroplasty in the elderly using systematic evaluation/Meta analysis method. MethodsThe researches on applying TEAS in hip replacement were systematically searched from PubMed, Cochrane library, Web of science, Embase, CBM, CNKI, VIP and Wanfang databases from the time of constructing database to October 7, 2020. Two researchers screened the papers simultaneously and extracted the data, assessed the quality of the included studies using Cochrane risk bias assessment tool and carried out Meta analysis adopting RevMan 5.3 software. ResultsA total of five randomized controlled trials (RCTs) were included, involving 344 subjects, the results of Meta analysis showed that the experiment group was lower than the control group in the decrease of MMSE one day and three days after operation, the incidence of POCD, the rise of TNF-α and IL-1β after the operation, and the difference had statistical meaning (P<0.05). The difference had no statistical meaning in the elevated level of IL-6 between both groups after the operation (P>0.05). ConclusionTEAS could lower the incidence of POCD in the elderly patients undergoing hip replacement and inhibit the release of inflammatory factors at early stage after the surgery.
