摘要:
目的分析慢性肺系疾病的中医内服膏方治疗的临床应用规律及优势病种施治用药规律。 方法检索中国知网及万方数据库2010—2020年关于膏方治疗慢性肺系疾病的临床应用文献，筛选符合纳入标准的文献，提取发表年份、服药季节、临床有效率、病名、方药组成等信息，标准化处理后进行统计分析。 结果共筛选出符合标准的文献164篇，2010—2020年文献研究总体呈上升趋势，服药季节包括冬季进服和四时可调；文献纳入164个膏方，主方共涉及423味中药，辅料涉及33味中药，胶类涉及5味中药；四气以温性、平性为主，五味以甘、辛、苦味为主，归经以肺、脾、肾经为主；高频中药有黄芪、茯苓、陈皮等；儿科膏方剂型以蜜膏、清膏为主，内科肺系疾病剂型以荤膏为主；优势病种为慢性阻塞性肺疾病、支气管哮喘等，多用于疾病稳定期。 结论膏方防治慢性肺系疾病种类广泛、临床研究成果显著，治疗以扶正固本补益为主、理气祛痰化瘀为辅。
Abstract:
ObjectiveTo analyze clinical application laws of oral ointment for the treatment of chronic pulmonary disease (CPD), the treatment and medication rules of clinical dominant diseases. MethodsThe literature on clinical application of ointment in the treatment of CPD were searched from CNKI and Wanfang between 2010 and 2020, and the papers meeting the inclusion criteria were screened to extract the information including the publication years, season of administration, clinical effective rate, the name of the disease, the composition of the prescription and the herbs, and to perform the statistical analysis after standardized processing. ResultsAll 164 papers meeting the inclusion criteria were screened out, and the literature studies between 2010 and 2020 were on the rise generally, the seasons of administration contained administration in winter and four seasons; the literature covered 164 ointment prescriptions, and the main prescriptions involved 423 herbs, the adjuvant materials contained 33 herbs, and the gels were five herbs; the herbs were warm and neutral mainly in four natures, belonging to sweet, pungent and bitter in five tastes, and entering the meridians of lung, spleen and kidney; high-frequency herbs were Huangqi (Astragali radix), Fuling [Poria cocos (Schw.)Wol] and Chenpi (Citrus reticulata Blanco); the forms of pediatric hebal pastes mainly contained honey paste and sugar-free paste mainly, and pastes for pulmonary diseases mostly contained animal products. The dominant diseases were chronic obstructive pulmonary disease, and bronchial asthma, and the herbal pastes were mostly used for the treatment of stable disease. ConclusionThe ointment could prevent and treat a wide range of chronic pulmonary diseases with remarkable clinical research results, taking enhancing healthy Qi, consolidating the body resistance and tonifying as the main therapies, regulating Qi, removing phlegm and resolving stagnation as the adjuvant methods.
