摘要:
目的探讨陇中整脊综合疗法治疗腰椎术后综合征（failed back surgery syndrome，FBSS）的临床疗效。 方法运用回顾性队列研究设计，收集60例FBSS患者，根据是否运用陇中整脊综合疗法分为综合疗法组（26例）和对照组（34例）。对照组予杜仲腰痛丸治疗，综合疗法组在对照组基础上予陇中整脊手法及功能锻炼指导，两组均治疗6个月。比较两组日本骨科协会评估治疗分数（Japanese orthopaedic association scores，JOA）、视觉模拟量表评分（visual analog score，VAS）及Oswestry功能障碍指数（oswestry disability index，ODI），并比较临床疗效。 结果两组患者VAS评分、ODI指数均随治疗时间呈降低趋势（P＜0.05），且综合疗法组下降趋势更明显（P＜0.05）；治疗6个月后，综合疗法组VAS评分、ODI指数均低于对照组（P＜0.05）。两组患者JOA评分均随治疗时间呈升高趋势（P＜0.05），且综合疗法组升高趋势更明显（P＜0.05）；治疗6个月后，综合疗法组JOA评分高于对照组（P＜0.05）。治疗6个月后，JOA改良率综合疗法组［73.08%（19/26）］优于对照组［38.24%（13/34）］（P＜0.05）。 结论陇中整脊综合疗法联合杜仲腰痛丸可有效改善FBSS患者功能障碍及腰部疼痛症状，且随治疗时间延长，效果更佳。
Abstract:
ObjectiveTo discuss clinical effects of Longzhong chiropractic comprehensive therapy in the treatment of failed back surgery syndrome (FBSS). MethodsRetrospective cohort study design was used to collect 60 FBSS patients, and they were separated into comprehensive therapy group (26 cases) and the control group (34 cases) according to whether they adopted Longzhong chiropractic comprehensive therapy or not. The control group took Duzhong lumbago pills, while Longzhong chiropractic comprehensive therapy and functional exercise guidance were administered to comprehensive therapy group based on the therapy of the control group, and both groups were treated for six months. To observe JOA, VAS and ODI between the two groups, and to compare clinical effects. ResultsVAS and ODI exhibited a declining trend with therapeutic time in two groups (P<0.05), and the downward trend of the comprehensive therapy group was more evident (P<0.05); after six months of the treatment, VAS and ODI of the comprehensive therapy group were lower than these of the control group (P<0.05). JOA presented an upward trend with therapeutic time in the two groups (P<0.05), and the rising trend of comprehensive therapy group was more noticeable (P<0.05); after six months of the treatment, JOA scores of comprehensive therapy group were higher than these of the control group (P<0.05). After six months of the treatment, JOA improvement rate of comprehensive therapy group reached [73.08%(19/26)], better than [38.24%(13/34)] of the control group (P<0.05). Conclusion Longzhong chiropractic comprehensive therapy and Duzhong lumbago pills could effectively improve dysfunction and lumbar pain in the treatment of FBSS, indicating a time-dependent enhancement of effect.
