摘要:
目的观察降脂化浊汤配合循经推拿治疗痰湿型非酒精性脂肪性肝病（non-alcoholic fatty liver disease，NAFLD）的临床疗效。 方法选择72例痰湿型NAFLD患者，按照治疗方法的不同分为对照组与治疗组，每组36例。两组均根据指南予非药物干预措施，对照组予阿托伐他汀钙片口服治疗，治疗组予降脂化浊汤配合循经推拿治疗，两组均连续治疗4周。比较两组治疗前后生化指标［谷草转氨酶（aspartate aminotransferase，AST）、丙氨酸转氨酶（alanine aminotransferase，ALT）、总胆固醇（total cholesterol，TC）、甘油三酯（triglyceride，TG）］、氧化应激指标［血清超氧化物歧化酶（superoxide dismutase，SOD）、谷胱甘肽过氧化物酶（glutathione peroxidase，GSH-Px）、总抗氧化能力（total antioxidant capacity，T-AOC）］及痰湿证单项症状评分的变化情况和临床疗效。 结果治疗后，两组患者血清AST、ALT、TC、TG水平均降低（P＜0.05），SOD、GSH-Px、T-AOC水平均升高（P＜0.05），痰湿证单项症状评分均减少（P＜0.05）；且治疗组上述指标改善程度均大于对照组（P＜0.05）。总有效率治疗组［94.44%（34/36）］高于对照组［72.22%（26/36）］（P＜0.05）。 结论降脂化浊汤配合循经推拿治疗痰湿型NAFLD疗效确切，能够抑制氧化应激反应，改善患者肝功能，调节血脂水平，改善临床症状。
Abstract:
ObjectiveTo observe clinical effects of lipid-lowering turbidity-resolving decoction and massage along meridians in the treatment of NAFLD with phlegm-dampness pattern. MethodsAll 72 patients were selected, and allocated to the control group and the treatment group in light of different therapeutic methods with 36 cases in each group. Both groups accepted non-pharmacological interventions according to the guideline, the control group took atorvastatin calcium tablets orally, while lipid-lowering turbidity-resolving decoction and massage along meridians were given to the treatment group, and both groups were treated for four weeks continuously. To compare biochemical indicators including AST, ALT, TC and TG, oxidative stress indicators containing SOD, GSH-Px and T-AOC, single symptom score for phlegm-dampness pattern, together with curative effects before and after the treatment in the two groups. ResultsAfter the treatment, the decrease in the levels of AST, ALT, TC and TG was found in the two groups (P<0.05), the levels of SOD, GSH-Px and T-AOC were increased (P<0.05), and single symptom score for phlegm-dampness pattern were reduced (P<0.05); and the improvements of the above indexes in the treatment group were greater than these of the control group (P<0.05). Total effective rate of the treatment group was [94.44%(34/36)], higher than [72.22%(26/36)] of the control group (P<0.05). ConclusionLipid-lowering turbidity-resolving decoction and massage along meridians demonstrates definite clinical effects in the treatment of NAFLD with phlegm-dampness pattern, it could inhibit oxidative stress response, enhance liver function, regulate the levels of blood lipid and improve clinical symptoms.
