摘要:
从“肺鼻同治”理论入手，探讨易蔚教授防治小儿支气管哮喘的临床经验与辨治思路，认为小儿支气管哮喘与过敏性鼻炎在解剖、经络、生理、病理及病机层面密切相关，呈现“肺鼻同病”状态，哮喘反复发作可诱发鼻炎；其次，急性期以“宣肺祛风、涤痰平喘”为法，采用龙脷平喘汤肺鼻同治，快速控制气道炎症与鼻部症状；第三，非急性期根据证型分别施治：痰甚者以三三二方平喘健脾化痰，风胜者以抗敏方祛风抗敏扶正，并联合穴位贴敷、小儿推拿、健康宣教等六位一体综合防治措施，以降低复发率、提高远期疗效。
Abstract:
Starting from the theory of "simultaneous treatment of lung and nose", this paper explores Professor Yi Wei’s clinical experience and diagnostic and therapeutic approach in preventing and treating pediatric bronchial asthma. It is proposed that pediatric bronchial asthma and allergic rhinitis are closely related in terms of anatomy, meridians, physiology, pathology, and pathogenesis, presenting a state of "simultaneous disorder of lung and nose". Moreover, recurrent asthma attacks may induce rhinitis. Second, during the acute stage, the method of "ventilating the lung, dispelling wind, clearing phlegm, and relieving asthma" is adopted, using Longli Pingchuan Decoction for simultaneous treatment of the lung and nose to rapidly control airway inflammation and nasal symptoms. Third, during the non-acute stage, treatment is tailored to different patterns: for those with excessive phlegm, the Sansan’er Formula is used to relieve asthma, strengthen the spleen, and resolve phlegm; for those with predominant wind, the Anti-Allergy Formula is applied to dispel wind, counteract allergies, and strengthen the body’s resistance. These are combined with six-in-one comprehensive preventive and therapeutic measures, including acupoint application, pediatric tuina (massage), and health education, to reduce the relapse rate and improve long-term outcomes.
