摘要:
自身免疫性溶血性贫血多由肝木失调、湿热熏蒸，脾肾亏虚、精血不足，正气虚弱、瘀血阻络所致。临床常表现为虚中夹实、本虚标实之病理机转和证侯。本虚为脾肾阳虚、气血亏损，标实为湿热内蕴、气机郁阻，或寒凝血脉、瘀血内阻。其主要病位在脾肾，涉及肝胆。临床辨证分为湿热内蕴、气血两虚、脾肾亏虚、瘀血阻络4型。用自拟抗溶系列方加减治疗的同时应注意正虚邪实、分清主次，中西结合、分期治疗，辨证论治，活用桂枝、柴胡、大黄、虎杖4药。
Abstract:
Autoimmune hemolytic anemia is mostly caused by liver dysfunction, dampness-heat fumigation, spleen-kidney deficiency, essence-blood deficiency, healthy-Qi deficiency and blood stasis obstructing the collaterals. It usually manifests deficiency accompanied with excess or deficiency in origin and excess in superficiality in clinic; it could be divided into spleen-kidney Yang deficiency, Qi-blood deficiency in the view of deficiency in origin, it could also be allocated to internal accumulation of dampness-heat, Qi stagnation, or blood accumulated in the blood vessels, internal obstruction of blood stasis in the view of excess in superficiality; the disease is located at spleen and kidney mainly, involved liver and gallbladder. It could be classified into four patterns in clinic: internal accumulation of dampness-heat, dual deficiency of Qi and blood, spleen-kidney deficiency and blood stasis obstructing the collaterals. Self-made antihemolytic prescriptions series are used to treat the disease. Simultaneously, in the treatment, we should pay attention to the deficiency or excess, primary or secondary causes, traditional Chinese medicine combined with Western medicine, treating according to the stages, syndrome differentiation and treatment, using GuiZhi (Cinnamomum cassia Presl), ChaiHu(Radix Bupleuri), DaHuang (Rheum palmatum L.) and HuZhang (Rhizoma Polygoni Cuspidati) flexibly.
