摘要:
目的：观察泻火降逆汤治疗肝胃郁热型胃食管反流病(GERD)的临床疗效及对胃肠激素的影响。方法：将100例患者采用随机法分为观察组和对照组各50例，对照组给予奥美拉唑肠溶胶囊和枸橼酸莫沙比利片口服，观察组在常规口服奥美拉唑肠溶胶囊的基础上，给予中药泻火降逆汤辨证加减治疗，2组均连续治疗8周，观察2组临床疗效、治疗前后证候积分和血清P物质(SP)、胆囊收缩素(CCK)的变化情况。结果：总有效率观察组为92.00%，对照组为64.00%，2组比较差异有统计学意义(P＜0.05)。证候积分2组治疗后各时间点比较，差异均有统计学意义(P＜0.05)。烧心、胸痛、反酸、腹胀、嗳气等症状的痊愈率2组比较，差异有统计学意义(P＜0.05)。血清SP及CCK治疗前后2组组内比较，差异有统计学意义(P＜0.05)，治疗后组间比较，差异也有统计学意义(P＜0.05)。结论：泻火降逆汤能较好地改善肝胃郁热证GERD患者的主要症状、体征，使SP、CCK等胃肠激素分泌趋于正常，且临床疗效显著。
Abstract:
Objective: To observe clinical effects of XieHuo JiangNi decoction in treating gastroesophageal reflux disease (GERD) of liver-stomach stagnant-heat pattern and its influence on gastrointestinal hormone. Methods: All 100 patients were allocated to the observation group and the control group by using randomized method, 50 cases each group, the control group took omeprazole enteric-coated capsules and mosapride citrate tablets orally, and the observation group were treated with modified XieHuo JiangNi decoction by syndrome differentiation on the foundation of oral administration of omeprazole enteric-coated capsules, both groups were treated for eight weeks consecutively, clinical effects, syndrome scales and the changes of serum P substances and cholecystokinin (CCK) in both groups were observed. Results: Total effective rate of the observation group was 92.00%, higher than 64.00% of the control group, and the difference had statistical meaning (P＜0.05). The difference was statistically significant in the comparisons of syndrome scales between both groups after treating at different times (P＜0.05). The difference showed statistical meaning in the comparisons of the cure rates of the symptoms including heartburn, chest pain, sour regurgitation, abdominal distension, belching and others (P＜0.05).The difference was statistically significant in the comparisons of the levels of SP and CCK before and after treating within two groups (P＜0.05), and there was a significant difference in the comparisons between both groups after treating (P＜0.05). Conclusion: XieHuo JiangNi decoction could improve main symptoms of GERD patients of liver-stomach stagnant heat pattern, make the secretion of SP and CCK normal and obtain notable effects.
