摘要:
目的：观察中药热敷治疗骨质疏松性胸腰椎压缩骨折经皮椎体后凸成形术(PKP)术后残留痛的临床疗效。方法：将骨质疏松性胸腰椎压缩骨折PKP术后残留痛患者90例按随机数字表法分为2组各45例，对照组给予依降钙素+钙尔奇D治疗；治疗组在对照组基础上行中药热敷。2组均随访3个月以上，记录治疗前、治疗2周后、治疗4周后以及末次随访时视觉模拟评分(Visual analogue scale，VAS)与Oswestry功能障碍指数(Oswestry disability index，ODI)及不良反应。结果：治疗前2组VAS评分、ODI指数比较差异无统计学意义(P＞0.05)；治疗后2组各时点VAS评分、ODI指数与治疗前比较均不同程度降低，差异有统计学意义(P＜0.05)；治疗后2组各时点VAS评分、ODI指数组间比较差异有统计学意义(P＜0.05)；治疗期间所有患者无相关干预措施的不良事件发生。结论：中药热敷可有效缓解骨质疏松性胸腰椎压缩骨折PKP术后残留痛，能促进腰背部功能康复，安全有效。
Abstract:
Objective: To explore clinical effects of traditional Chinese medicine fomentation in treating the residual pain of osteoporotic thoracolumbar compression fracture (OTCF) after percutaneous kyphoplasty (PKP). Methods: All 90 patients were allocated to two groups according to random number table method, the control group were given calcitonin(CT) and Caltrate D; the treatment group accepted TCM fomentation on the foundation of the therapy the control group received. Both groups were followed for more than three months, adverse reaction, Oswestry disability index (ODI) and visual analogue scale(VAS) of both groups were recorded before treating, in two weeks and four weeks after treating and the last follow-up. Results: The difference had no statistical meaning in the comparisons of VAS and ODI between both groups before the treating(P＞0.05); VAS and ODI at different times after treating lowered compared with before treating in both groups, and the difference had statistical meaning (P＜0.05); the difference was statistically significant in the comparisons of VAS and ODI between both groups after treating (P＜0.05). No adverse events related to the intervention measures happened during therapeutic period. Conclusion: TCM fomentation could effectively mitigate the residual pain of OTCF after PKP, promote functional recovery of the lower back, which is safe and effective.
