摘要:
目的：探讨血清脂蛋白α[Lp(α)]及胱抑素C（CysC）在糖尿病早期肾功能损害与中医证型间的相关性。方法：将302例2型糖尿病患者按尿微量白蛋白排泄率分为单纯糖尿病组(A组)、早期糖尿病肾病组(B组)，选取本院体检中心120例体检健康者作为对照组；对早期糖尿病肾病按中医证型分组，对各组指标进行统计对比分析。结果：B组血清Lp(α)、CysC升高明显，与A组和对照组比较，差异有统计学意义(P＜0.05)；B组中气阴两虚证发生率最高(46.75%)，其次为阴虚燥热证(33.13%)及脾虚湿困证(20.12%)，3证型与对照组比较Lp(α)、CysC升高明显，差异有统计学意义(P＜0.05)，阴虚燥热组和气阴两虚组患者的CysC差异无统计学意义(P＞0.05)，脾虚湿困组与阴虚燥热和气阴两虚组比较CysC明显升高，差异有统计学意义(P＜0.05)，3证型间Lp(α)比较，差异无统计学意义(P＞0.05)。结论：在糖尿病早期肾功能损害患者血清中Lp(α)、CysC升高明显，CysC水平与中医证型可能存在一定的相关性，可作为早期糖尿病肾病诊断和中医辨证分型的参考指标。
Abstract:
Objective: To survey the correlation of lipoprotein α[ Lp(α)], cystatin C (CysC) with early renal function damage of diabetes and TCM patterns. Methods: All 302 patients with type two diabetes mellitus (T2DM) were assigned to single DM group (group A) and early DN group (group B) in light of urinary albumin excretion rate, and 120 healthy people who accepted physical examination in medical examination center of our hospital were chosen as the control group; the patients in group B were classified according to TCM patterns, and the indexes in different groups were counted, compared and analyzed. Results: The levels of Lp(α) and CysC rose notably in group B, the difference was statistically significant when it was compared with group A and the control group (P＜0.05); the incidences of Qi-Yin dual deficiency pattern in group B were the highest (46.75%), the next were Yin-deficiency dryness-heat pattern (33.13%) and spleen-deficiency fluid-retention pattern (20.12%), the levels of Lp(α) and CysC raised obviously when three patterns were compared with the control group, and the difference showed statistical meaning (P＜0.05), the difference had no statistical meaning in the comparisons of CysC between Yin-deficiency dryness-heat pattern and Qi-Yin dual deficiency pattern (P＞0.05), the level of CysC raised remarkably when the group of spleen deficiency fluid-retention and Yin deficiency dryness-heat were compared with Qi-Yin dual deficiency group, and the difference showed statistical meaning (P＜0.05), the difference had no statistical meaning in the comparisons of Lp(α) among three patterns (P＞0.05). Conclusion: The levels of Lp(α) and CysC rise in the serum of the patients who are suffering early renal function damage of diabetes, the levels of CysC might be related to TCM patterns to a certain degree, and it could be taken as the parameters for the diagnosis of early DN and syndrome differentiation and patterns.
