摘要:
目的：通过分析某三级甲等中医院静脉药物配置中心(pharmacy intravenous admixture service，PIVAS)中药注射剂不合理医嘱情况，为临床合理使用中药注射剂提供参考。方法：收集2017年3～8月某三甲中医院PIVAS中药注射剂医嘱18 352份，对其中不合理医嘱按分析指标进行汇总、统计和分析。结果：PIVAS中药注射剂不合理医嘱1567份，不合理医嘱占总医嘱份数的8.54%(1567/18 352)。不合理医嘱主要为溶媒及溶媒量选择不当，占不合理医嘱的63.18%(1990/1567)；其次为单次用药剂量不当和超适应症用药,分别为19.53%(306/1567)和16.46%(258/1567)。单次用药剂量不合理医嘱以超过说明书用药剂量用药为主。另有极少数用药途径不合理医嘱和与胰岛素配伍不当医嘱。结论：我院PIVAS中药注射剂存在不合理医嘱，药师应加强中药注射剂的审方，阻止不合理医嘱通过，最大限度降低中药注射剂不合理使用几率，促进安全有效使用中药注射剂。
Abstract:
Objective: To provide the reference for the rational use of TCM injection in clinic by analyzing the unreasonable prescription of TCM injection in PIVAS of a tertiary first - class TCM hospital. Methods: After collecting 18 352 prescriptions of TCM injections in PIVAS from March to August, 2017, unreasonable prescriptions were gathered, counted and analyzed in light of the indexes. Results: There were 1567 unreasonable prescriptions of TCM injection in PIVAS, and they held 8.54%(1567/18 532) of total medical advices. Unreasonable medical advices were improper selection of solvent and solvent amount, and they occupied 63.18%(1990/1567) of unreasonable medical advices; the next were improper single dose and superindication medication, 19.53%(306/1567) and 16.46%(258/1567) respectively. Unreasonable single dose was beyond the dose in the instructions mainly. Besides there were very few unreasonable administration routes and improper insulin compatibility. Conclusion: Unreasonable medical advices of TCM injections exist in PIVAS, the pharmacists should enhance the check of TCM injection, prevent the verification of unreasonable mecical advices, decrease the rational use rate of TCM injections at maximum and promote safe and effective use of TCM injections.
