摘要:
目的：探讨益气养阴通络方对早期糖尿病肾病患者炎症因子及肾功能的影响。方法：将早期糖尿病肾病患者98例根据随机数字表法分为对照组和观察组各49例。对照组采用缬沙坦治疗，观察组在对照组基础上采用益气养阴通络方治疗，观察两组患者炎症因子水平、肾功能指标、中医证候积分及不良反应情况。结果：观察组治疗后C-反应蛋白(C-reaction protein，CRP)、肿瘤坏死因子α(tumor necrosis factor α，TNF-α)、白细胞介素6(interleukin-6，IL-6)及白细胞介素8(interleukin-8，IL-8)水平均低于对照组，差异有统计学意义(P＜0.05)；观察组治疗后尿微量白蛋白(m-ALB)、血清胱抑素C(Cys-C)及β2微球蛋白(β2-MG)水平均低于对照组，差异有统计学意义(P＜0.05)；观察组患者治疗后气短、尿浊、神疲乏力、咽干口燥及肢体浮肿评分均低于对照组，差异有统计学意义(P＜0.05)；观察组不良反应发生率与对照组比较差异无统计学意义(P＞0.05)。结论：益气养阴通络方与缬沙坦联用可有效减轻早期糖尿病肾病患者炎症因子水平，改善肾功能及临床症状，且安全性较高。
Abstract:
Objective: To explore the influence of Qi-invigorating Yin-nourishing collateral-dredging prescription(QYCP) on renal function and inflammatory factors of the patients with early diabetic nephropathy(DN). Methods: All 98 early DN patients were allocated to the control group and the observation group according to random number table method, 49 cases each group. The control group were treated by valsartan, and the observation group by QYCP based on valsartan, to observe the levels of inflammatory factors, renal function indexes, TCM syndrome integrals and adverse reaction between both groups. Results: The observation group was lower than the control group in the levels of CRP, TNF-α, IL-6 and IL-8, and the difference had statistical meaning (P＜0.05); the observation group was lower than the control group in m-ALB, Cys-C and β2-MG after treating, and the difference was statistically significant(P＜0.05); the observation group was lower than the control group in the scores of panting, turbid urine, exhausted, dry throat and mouth and limb swelling after treating, and the difference was statistically significant (P＜0.05); the difference had no statistical meaning when the observation group was compared with the control group in the incidence of adverse reaction (P＞0.05). Conclusion: QYCP combined with valsartan could effectively lower the levels of inflammatory factors of the patients with early DN, improve renal function and clinical symptoms with higher safety.
