摘要:
目的:观察自拟平喘煎联合特布他林雾化吸入治疗婴幼儿喘息性支气管炎急性发作期(外寒内热、痰热壅肺证)的临床效果,探讨其对外周血白细胞介素17(interleukin-17,IL-17)、可溶性细胞间黏附分子1(soluble intercellular adhesion molecule-1,sICAM-1)、干扰素γ(interferon γ,IFN-γ)水平的影响。方法:将喘息性支气管炎急性发作期(外寒内热、痰热壅肺证)婴幼儿100例按照随机数字表法分为对照组和观察组各50例。对照组患儿给予特布他林雾化吸入,观察组患者在对照组基础上用自拟平喘煎治疗,连续治疗4周后观察两组患儿喘息、咳嗽、肺部喘鸣音等主要临床症状消失时间,中医证候积分及血清IL-17、sICAM-1、IFN-γ水平变化情况。结果:有效率对照组为78.0%(39/50),低于观察组的92.0%(46/50)(P<0.05);观察组治疗后喘息、咳嗽、肺部喘鸣音等主要临床症状消失时间较对照组短(P<0.05);观察组治疗后呼吸道症状中医证候积分低于对照组(P<0.05);肺功能指标最大呼气峰流速(peak expiratory flow rate,PEF)、肺活量(forced vital capacity,FVC)、第一秒用力呼气量(forced expiratory volume in one second,FEV1)、FEV1/FVC、呼吸系统静态顺应性(Crs SO)水平高于对照组,呼吸系统阻力(Rrs SO)较对照组降低(P<0.05);血清IL-17、sICAM-1水平低于对照组,IFN-γ水平高于对照组(P<0.05)。结论:自拟平喘煎联合特布他林雾化吸入有利于减轻喘息性支气管炎急性发作期患儿的临床症状,促进症状消失,改善肺功能,其机制可能在于通过调控血清IL-17、sICAM-1、IFN-γ水平减轻炎性反应所致的气道损伤,调节Thl/Th2免疫平衡。
Abstract:
Objective: To investigate the clinical efficacy of self-made empirical formula Pingchuan Jian in combination with atomization inhalation of terbutaline in the treatment of acute exacerbation of infantile asthmatic bronchitis (external cold and internal heat, phlegm-heat blocking lung pattern), and explore the influence on the levels of IL-17, sICAM-1 and IFN-γ in peripheral blood. Methods: One hundred infants were allocated to the control group and the observation group according to random number table method, 50 cases each group. The control group accepted atomization inhalation of terbutaline, and the observation group took self-made empirical formula Pingchuan Jian based on Western medicine, to observe disappearing time of main clinical symptoms including panting, cough, lung wheezing and others, TCM syndrome integrals and the levels of IL-17, sICAM-1 and IFN-γ of the children in both groups after treating four consecutive weeks. Results: The effective rate of the control group was 78.0%(39/50), lower than 92.0%(46/50) of the observation group(P<0.05); disappearing time of main clinical symptom including panting, cough, lung wheezing and others in the observation group was quicker than that of the control group after treating notably (P<0.05); the observation group was lower than the control group in TCM syndrome integrals of respiratory tract after treating(P<0.05); lung function indexes such as PEF, FVC, FEV1, FEV1/FVC and Crs SO of the observation group were higher than these of the control group, Rrs SO reduced than the control group(P<0.05); the levels of IL-17 and sICAM-1 were lower than these of the control group, IFN-γ level was higher than that of the control group (P<0.05). Conclusion: Self-made empirical formula Pingchuan Jian in combination with atomization inhalation of terbutaline could help alleviate clinical symptoms of the infants, promote symptoms disappearing, improve lung function with definite clinical effects, and its mechanism might be through regulating the levels of IL-17, sICAM-1 and IFN-γ to relieve airway injury induced by inflammatory response, and to adjust Thl/Th2 immune balance.
