摘要:
目的：探讨血必净注射液联合替加环素对碳青霉烯耐鲍曼不动杆菌感染的临床效果。方法：将泛耐药鲍曼不动杆菌肺部感染患者66例随机分为对照组与研究组各33例。对照组予替加环素，首剂100 mg，后50 mg，静脉滴注，每12 h 1次，维持治疗14天；研究组加用血必净注射液50 mL，每日两次，维持治疗14天。检测两组患者血清白细胞(white blood cell，WBC)、C反应蛋白(C-reaction protein，CRP)、降钙素原(procalcitonin,PCT)水平，并进行细菌学鉴定和药敏试验，评价细菌学疗效以及临床疗效。结果：与治疗前比较，治疗后两组患者WBC、CRP、PCT水平均降低(P＜0.01)，研究组低于对照组(P＜0.01)；细菌清除率对照组为30.3%(10/33)，研究组为54.5%(18/33)，两组比较差异有统计学意义( χ2=3.970,P=0.046)；总有效率对照组为51.5%(17/33)，研究组为75.8%(25/33)，两组比较差异有统计学意义( χ2=4.190,P=0.041)。结论：血必净注射液联合替加环素对碳青霉烯泛耐药鲍曼不动杆菌感染疗效优于单纯替加环素。
Abstract:
Objective: To explore clinical effects of Xuebijing injection combined with tigecycline in treating the patients suffering carbapenem resistant acinetobacter baumannii infection. Methods: All 66 patients were randomized into the control group and the study group, 33 cases each group. The control group were given tigecycline, initial dose 100mg, later 50 mg, intravenous dripping, once every 12 hours, maintenance therapy for 14 days; the study group accepted Xuebijing injection, 50mL, twice each day, maintenance therapy for 14 days. To detect the levels of PCT, CRP and WBC between both groups, to perform bacteriological identification and drug sensitive test, and to assess clinical effects and curative effects of bacteriology. Results: Compared with before treating, the levels of WBC, CRP and PCT of both groups reduced after treating(P＜0.01), the study group was lower than the control group(P＜0.01), bacterial clearance rate of the control group was 30.3%(10/33), lower than 54.5%(18/33) of the study group, the difference had statistical meaning between both groups( χ2=3.970, P=0.046); total effective rate of the control group was 51.5%(17/33), lower than 75.8%(25/33) of the study group, and the difference showed statistical meaning between both groups( χ2=4.190, P=0.041). Conclusion: Xuebijing injection combined with tigecycline are superior to only tigecycline in treating carbapenem resistant acinetobacter baumannii infection.
