目的探讨维持性血液透析患者自我感受负担现状及相关因素。 方法选择2017年3月至2018年3月行维持性血液透析（maintenance hemodialysis，MHD）患者137例作为研究对象。采用Likert 5级评分法，用单因素和多因素分析MHD患者自我感受负担（self-perceived burden，SPB）影响因素。 结果137例MHD患者中，无明显自我感受负担28例（20.44%），轻度SBP 59例（43.06%），中度SBP 34例（24.81%），重度SBP 16例（11.68%）。MHD患者中，经济负担维度均分最高，其次为身体负担和情感负担。经单因素分析表明，性别、文化程度、合并高血压、合并糖尿病和基础疾病SPB量表评分比较差异无统计学意义（P＞0.05）；年龄、透析龄、透析频率、照顾者身体状况和生活自理能力SPB量表评分比较差异有统计学意义（P＜0.05）。将上述单因素分析具有统计学差异的纳入多因素分析中，年龄、透析龄、透析频率、照顾者身体状况和生活自理能力为影响MHD患者SPB危险因素。 结论大部分MHD患者存在轻度以上自我感受负担，年龄、透析龄、透析频率、照顾者身体状况和生活自理能力为影响MHD患者SPB的危险因素。
Abstract:
ObjectiveTo explore the related factors and current situations of self-perceived burden (SPB) in maintenance hemodialysis (MHD) patients. MethodsAll 137 patients who accepted MHD and were hospitalized from March, 2017 to March, 2018 were chosen as the research targets. univariate and multivariate analysis were used to survey the influencing factors of SPB in MHD patients by using Likert 5-grade scoring method. ResultsAmong 137 MHD patients, 28 ones without SPB held 20.44%, 59 cases of mild SPB 43.06%, 34 cases of moderate SPB 24.81%, 16 cases of severe SPB 11.68%. Financial burden dimensional scores were the highest, physical and emotional burdens were the next. No statistical difference could be seen in the comparisons of SPB scales such as gender, eudcationcal degrees, complicated with hypertension, complicated with diabetes mellitus, basic disease by univariate analysis (P>0.05), the comparison of age, dialysis age, dialysis frequency, caregiver′s physical condition and self-care ability showed the statistical difference (P<0.05). Age, dialysis age, dialysis frequency, caregiver′s physical condition and self-care ability have become the risk factors of SPB in MHD patients after including these with statistical difference in the above univariate analysis in the multivariate analysis. ConclusionMost MHD patients suffer mild SPB or above, age, dialysis age, dialysis frequency, caregiver′s physical condition and self-care ability have become the risk factors of SPB in MHD patients.
