摘要:
目的探究寒喘祖帕颗粒配合常规疗法对异常黏液质性哮喘患者咳嗽症状、肺功能及血清P物质(substance P,SP)、嗜酸粒细胞阳离子蛋白(eosinophil cationic protein,ECP)、神经营养素3(neurotrophin-3,NT-3)的影响。 方法选取异常黏液质性哮喘患者106例,按照随机数字表法分为观察组和对照组,各53例。对照组采取常规疗法,观察组在对照组基础上给予寒喘祖帕颗粒。比较两组治疗效果、治疗前后咳嗽症状积分、肺功能[用力肺活量(forced vital capacity,FVC)、第1秒呼气容积(forced expiratory volume in one second,FEV1)、FEV1/FVC、呼气峰流速(peak expiratory flow rate,PEF)]、气道炎症反应(SP、ECP、NT-3)、T淋巴细胞亚群(CD3+、CD4+、CD4+/CD8+)及不良反应发生情况。 结果观察组总有效率[94.34%(50/53)]高于对照组[81.13%(43/53)](P<0.05)。观察组气短、胸闷、咯痰、喘息、哮鸣音消失时间短于对照组(P<0.05)。治疗4、8周后观察组日间咳嗽积分、夜间咳嗽积分、总积分,血清SP、ECP、NT-3水平低于对照组(P<0.05),FVC、FEV1、FEV1/FVC、PEF、CD3+、CD4+、CD4+/CD8+水平高于对照组(P<0.05)。观察组不良反应发生率[9.43%(5/53)]与对照组[5.66%(3/53)]相比,差异无统计学意义(P>0.05)。 结论寒喘祖帕颗粒配合常规疗法治疗异常黏液质性哮喘,能有效抑制气道炎症反应,调节细胞免疫功能,改善肺功能,减轻咳嗽症状,缩短症状改善时间,临床效果显著。
Abstract:
ObjectiveTo investigate the effects of Hanchuan Zupa granules combined with conventional therapy on cough, lung function and the levels of serum substance P (SP), eosinophil cationic protein (ECP), neurotrophin-3 (NT-3) in patients with abnormal mucinous asthma. MethodsAll 106 patients were allocated to the observation group and the control group according to random number table method, with 53 cases in each group. The control group adopted conventional therapy, and the observation group took Hanchuan Zupa granules on the foundation of routine therapy. To compare therapeutic effects, cough symptom scores before and after the treatment, lung function [FVC, FEV1, FEV1/FVC and PEF], airway inflammatory response (SP, ECP and NT-3), T lymphocyte subsets (CD3+, CD4+, CD4+/CD8+) and the incidence of adverse reaction between both groups. ResultsTotal effective rate of the observation group was [94.34%(50/53)], higher than [81.13%(43/53)] of the control group (P<0.05). The observation group was shorter than the control group in the disappearing time of panting, chest distress, expectoration, gasping and wheezing rale (P<0.05). After four and eight weeks of the treatment, daily cough scores, night cough scores, total scores, the levels of SP, ECP and NT-3 of the observation group were lower than these of the control group (P<0.05), FVC, FEV1, FEV1/FVC, PEF, CD3+, CD4+, CD4+/CD8+ higher than these of the control group (P<0.05). The difference had no statistical meaning in the incidence of adverse reaction between the observation group [9.43%(5/53)] and the control group [5.66%(3/53)] (P>0.05). ConclusionHanchuan Zupa granules combined with conventional therapy in the treatment of abnormal mucinous asthma could effectively inhibit airway inflammatory response, regulate cellular immune function, and improve pulmonary function, relieve cough, shorten the time of improving symptoms with remarkable clinical effects.
