摘要:
目的观察不完全脊髓损伤患者应用调跷通督益髓汤联合脑机接口（brain computer interface，BCI）训练的疗效。 方法选取102例不完全脊髓损伤患者，采用随机数字表法分为对照组和观察组，每组51例。对照组接受传统综合康复训练联合调跷通督益髓汤治疗，观察组在对照组基础上加用BCI训练，两组共治疗8周，于治疗后第3、6月进行随访。比较两组脊髓损伤ASIA运动及感觉指数评分，神经生长因子（nerve growth factor，NGF）、脑源性神经营养因子（brain derived neurotrophic factor，BDNF）水平，神经功能及中医症状评分。 结果治疗后第1、3、6个月后，观察组ASIA运动指数、感觉指数评分及NGF、BDNF水平均高于对照组（P＜0.05）。治疗后第6个月，观察组肌力优于对照组，神经功能分级差异有统计学意义（P＜0.05）；两组腰椎曲度分级评分差异无统计学意义（P＞0.05）；观察组行走、麻木不仁、大便失禁、小便失禁、下肢肌肉拘挛、疼痛评分均低于对照组（P＜0.05）。 结论调跷通督益髓汤联合BCI训练可促进不完全脊髓损伤患者神经功能因子水平恢复，改善肌肉、感觉功能及临床症状，提升预后。
Abstract:
ObjectiveTo survey therapeutic effects of Tiaoqiao Tongdu Yisui (regulating-heel vessel unobstructing-governor meridian benefiting-marrow) decoction combined with BCI for the patients suffering incomplete spinal cord injury (SCI). MethodsA total of 102 patients suffering incomplete SCI were chosen, and divided into the control group and the observation group using random number table method with 51 cases in each. The control group was treated with traditional comprehensive rehabilitation training and Tiaoqiao Tongdu Yisui decoction while BCI training was administered to the observation group based on the therapy of the control group, and both groups were treated for eight weeks, and follow-up was conducted at the 3rd and 6th months after treatment. To compare American Spinal Injury Association (ASIA) motor and sensory scores, the levels of NGF and BDNF, neurological function and TCM symptom scores between the two groups. ResultsASIA motor and sensory scores, the levels of NGF and BDNF in the observation group were higher than these in the control group after treatment at one, three, and six months (P<0.05). After treatment at six months, muscle force in the observation group was better than that of the control group, the difference was statistically significant in neurological function grading (P<0.05); the difference was not statistically significant in lumbar lordosis grading score between the two groups (P>0.05); the observation group was lower than the control group in the scores of walking, numbness, fecal incontinence, urinary incontinence, lower limb muscle cramps and VAS scores (P<0.05). ConclusionTiaoqiao Tongdu Yisui decoction combined with BCI training for the patients suffering incomplete SCI could promote the recovery of neurological function factors, improve muscle, sensory function and clinical symptoms, and enhance the prognosis.
