摘要:
目的探讨握力和体力活动对维持性血液透析（maintenance hemodialysis，MHD）患者营养状况的预测价值。 方法选取规律进行MHD治疗的286名患者进行调查，收集患者一般资料、实验室资料、透析相关资料、人体测量学指标、营养状况和体力活动资料，并对相关指标进行分析。 结果调查患者中营养不良者占19.2%（55/286），营养不良炎症评分（malnutrition inflammation score，MIS）和握力、人类活动概况量表（human activity profile，HAP）各维度得分及校正体力活动得分（adjusted activity score，AAS）之间均呈显著负相关（P＜0.05）。AAS（OR=0.982，95%CI：0.968～0.996，P=0.011）、BMI值（OR=0.875，95%CI：0.783～0.977，P=0.018）和上臂中围（OR=0.838，95%CI：0.758～0.926，P=0.001）是MHD患者营养状况的独立预测因子。 结论体力活动和体质指数是MHD患者营养状况的独立预测指标，且是营养状况的保护因素，患者体力活动能力和体质指数越高，营养状况越好。
Abstract:
ObjectiveTo investigate the predictive value of handgrip strength and physical activity for the nutritional status of maintenance hemodialysis (MHD) patients. MethodsA total of 286 patients undergoing regular MHD were selected for the study, to collect general information, laboratory data, dialysis-related data, anthropometric indicators, nutritional status, and physical activity data, and to analyze the relevant indicators. ResultsAmong the surveyed patients, those with malnutrition accounted for 19.2% (55/286). MIS was significantly negatively correlated with handgrip strength, scores of all dimensions of the Human Activity Profile (HAP), and AAS (P<0.05). AAS (OR=0.982, 95%CI: 0.968-0.996, P=0.011), BMI value (OR=0.875, 95%CI:0.783-0.977, P=0.018) and mid-upper arm circumference (OR=0.838, 95%CI: 0.758-0.926, P=0.001) are independent predictors of nutritional status in MHD patients. ConclusionPhysical activity and body mass index (BMI) are independent predictors of nutritional status in MHD patients, and protective factors for nutritional status, The higher the patient’s physical activity capacity and BMI, the better the nutritional status.
