目的探究浮针优势病种及应用现状,以更好指导临床和推广使用。 方法检索中国知网(CNKI)、万方(WF)、维普(VIP)数据库,筛选全部含有“浮针”的文献,运用文献计量学方法统计年度发表数量、病种分类,筛选浮针独立干预的随机对照试验(randomized controlled trial,RCT)文献≥5篇确定为优势病种,并对浮针独立干预且RCT≥10篇的病种采用改良的Jadad量表和Cochrane偏倚风险评估工具进行质量评价。 结果共筛选出临床文献899篇,浮针独立干预RCT文章135篇,最终纳入浮针独立干预且RCT≥10篇的3个病种共计41篇RCT进行循证评价。浮针适应症覆盖7个疾病系统,达57种,优势病种9个,集中在肌肉骨骼和结缔组织系统的颈椎病、肩周炎、腰椎间盘突出症、膝骨性关节炎、肱骨外上髁炎、肌筋膜炎,神经系统的中风病,以及其他类别中的医源性术后疼痛、带状疱疹后遗神经痛。 结论现阶段浮针适应症较为广泛,优势病种较为集中,以颈椎病、腰椎间盘突出症、膝骨性关节炎为代表,通过文献循证评价发现,相关研究普遍存在文献质量偏低、风险偏倚不确定的现象,建议今后提高试验设计的规范性和严谨性。
Abstract:
ObjectiveTo explore the dominant diseases and current application status of Fu’s subcutaneous needling, in order to better guide clinical practice and promote its widespread use. MethodsThe China National Knowledge Infrastructure (CNKI), Wanfang, and VIP databases were searched to retrieve all literature containing the term "Fu’s subcutaneous needling." Bibliometric methods were used to analyze the annual number of publications and disease classification. Randomized controlled trial (RCT) literature involving Fu’s subcutaneous needling as an independent intervention was screened, and diseases with five or more RCTs were identified as dominant diseases. For those diseases with ten or more RCTs of independent Fu’s subcutaneous needling intervention, quality assessment was conducted using the modified Jadad Scale and the Cochrane Risk of Bias Tool. ResultsA total of 899 clinical articles were screened, among which 135 were randomized controlled trials (RCTs) involving Fu’s subcutaneous needling as an independent intervention. Ultimately, 41 RCTs covering three diseases, each with 10 or more RCTs of independent Fu’s subcutaneous needling intervention, were included for evidence-based evaluation. Fu’s subcutaneous needling is indicated for 57 diseases across 7 disease systems, with 9 dominant diseases. These are primarily concentrated in the musculoskeletal and connective tissue system, including cervical spondylosis, frozen shoulder (adhesive capsulitis), lumbar disc herniation, knee osteoarthritis, lateral epicondylitis of the humerus (tennis elbow), and myofasciitis; in the nervous system, including stroke; and in other categories, including iatrogenic postoperative pain and postherpetic neuralgia. ConclusionAt present, Fu’s subcutaneous needling has a relatively wide range of indications, with its dominant diseases being relatively concentrated, particularly represented by cervical spondylosis, lumbar disc herniation, and knee osteoarthritis. Evidence-based evaluation of the literature reveals that related studies generally suffer from low literature quality and unclear risk of bias. It is recommended that future research improve the standardization and rigor of trial design.
