摘要:
目的运用数据挖掘技术分析推拿治疗脑卒中后痉挛状态的取穴规律。 方法选择中国期刊全文数据库（China national knowledge infrastructure，CNKI）、万方数据库、维普中文科技期刊数据库（China science and technology journal database，CSTJ）、PubMed及中国生物医学文献服务系统中推拿治疗脑卒中后痉挛状态的临床文献并进行筛选，采用Excel 2020整理符合标准文献中的推拿取穴处方，分析穴位使用频次；将数据导入SPSS Statistics 25.0软件对高频腧穴进行描述性分析与聚类分析；采用SPSS Modeler 18.0软件，运用Apriori算法对高频腧穴进行关联规则分析并构建关联网络。 结果共纳入170篇文献，推拿治疗脑卒中后痉挛状态常用主穴为曲池、合谷、手三里、阳陵泉、足三里、肩髃；穴位选取以四肢部为主；经络选用以足太阳膀胱经居多；经关联规则分析共得134条关联规则，均为3个穴位组合；对21个高频穴位聚类分析可划分为5类。 结论推拿治疗脑卒中后痉挛状态的取穴具有一定规律，以选取四肢部穴位为主，侧重选用足太阳膀胱经腧穴。
Abstract:
ObjectiveTo analyze the law of acupoint selection of massage in the treatment of spastic state after stroke by using data mining technology. MethodsClinical literature on the treatment of post-stroke spasticity with massage was selected and screened from the China National Knowledge Infrastructure (CNKI), Wanfang Database, China science and technology journal database (CSTJ), PubMed, and SinoMed. Massage acupoint prescriptions from the eligible literature were compiled using Excel 2020, to analyze the frequency of acupoint use. The data were then imported into SPSS Statistics 25.0 software for descriptive analysis and cluster analysis of high-frequency acupoints. Additionally, association rule analysis on high-frequency acupoints was performed using the Apriori algorithm in SPSS Modeler 18.0 software, to construct an association network. ResultsA total of 170 articles were included. The most commonly used main acupoints in massage treatment for post-stroke spasticity were Quchi (LI11), Hegu (LI4), Shousanli (LI10), Yanglingquan (GB34), Zusanli (ST36), and Jianyu (LI15). The selected acupoints were predominantly located on the limbs. The Bladder Meridian of Foot-Taiyang (BL) was the most frequently used meridian. Association rule analysis yielded a total of 134 association rules, all of which were combinations of three acupoints. Cluster analysis of 21 high-frequency acupoints classified them into five clusters. ConclusionThe acupoint selection in massage treatment for post-stroke spasticity follows certain rules, primarily focusing on acupoints located on the limbs, with particular emphasis on the acupoints of the Bladder Meridian of Foot-Taiyang.
