目的探析特应性皮炎(atopic dermatitis,AD)中医药临床用药规律,挖掘核心药物配伍组合,阐明中药治疗AD组方内在机制。 方法检索中国知网、万方、维普等数据库近十年中医药治疗特应性皮炎相关文献,严格按照纳入排除标准筛选文献,提取内服方剂药物信息;运用Excel统计药物使用频次、性味归经、功效分类,采用SPSS软件对高频药材开展数理统计分析。 结果共纳入25篇有效文献,涉及中药96味,药物总频次280次;茯苓、白术、生地黄、当归、甘草、白鲜皮、防风、地肤子等15味为临床高频常用药;药物功效以清热、补虚、解表、利水渗湿为主,药性偏寒、温,药味多甘、苦、辛,归经集中于肝、肺、脾、胃经。 结论中医药治疗特应性皮炎以理气健脾、滋阴润肤、清热利湿为核心治则,临床常用参苓白术散、健脾祛风汤、当归饮子等方剂,多从肝、肺、脾、胃四经辨证施治,用药配伍规律明确,可为临床遣方用药提供参考依据。
Abstract:
ObjectiveTo explore the clinical medication patterns of traditional Chinese medicine (TCM) in the treatment of atopic dermatitis (AD), identify core herb-pair combinations, and elucidate the intrinsic mechanisms underlying TCM formula composition for this disease. MethodsRelevant literature on TCM treatment of AD published in the past decade was retrieved from databases including China National Knowledge Infrastructure (CNKI), Wanfang Data, and VIP Chinese Science and Technology Periodical Database. The retrieved studies were screened according to strict inclusion and exclusion criteria, and information on orally administered TCM formulas was extracted. The frequency of herb use, their properties, flavors, meridian tropisms, and efficacy classifications were statistically analyzed using Excel. Furthermore, mathematical and statistical analyses were performed on high-frequency herbs using SPSS software. ResultsA total of 25 eligible studies were included, involving 96 Chinese medicinal herbs with a cumulative frequency of 280 occurrences. Fifteen herbs, including Fuling (Poria), Baizhu (Atractylodes macrocephala), Shengdihuang (Rehmannia glutinosa), Danggui (Angelica sinensis), Gancao (Glycyrrhiza uralensis), Baixianpi(Dictamnus dasycarpus), Fangfeng(Saposhnikovia divaricata), and Difuzi(Kochia scoparia), were identified as the most frequently used clinical herbs. In terms of therapeutic efficacy, the herbs were primarily attributed to heat-clearing, deficiency-tonifying, exterior-releasing, and dampness-draining categories. The predominant properties were cold and warm, with flavors being mainly sweet, bitter, and pungent. The affected meridian tropisms were mainly the Liver, Lung, Spleen, and Stomach meridians. ConclusionThe core therapeutic principles of TCM in treating atopic dermatitis are regulating Qi and strengthening the spleen, nourishing Yin and moisturizing the skin, and clearing heat and draining dampness. Commonly used clinical formulas include Shenling Baizhu San, Jianpi Qufeng Tang, and Danggui Yinzi. Treatment is predominantly based on syndrome differentiation targeting the liver, lung, spleen, and stomach meridians. The evident compatibility patterns of the herbal formulas provide a reference for clinical prescription and medication.
