摘要:
目的观察天芎镇痛颗粒治疗肝风上扰型偏头痛的临床疗效。 方法选取肝风上扰型偏头痛患者120例，按照随机数字表法分为观察组与对照组，每组60例。对照组予盐酸氟桂利嗪胶囊治疗，观察组予天芎镇痛颗粒治疗，疗程均为30天。比较两组治疗前后头痛发作次数、持续时间及头痛程度，中医证候评分，大脑中动脉、大脑前动脉、大脑后动脉的收缩期血流速度峰值（peak systolic blood flow velocity，Vs）异常比率及临床疗效。 结果治疗后，两组患者头痛发作次数、持续时间、头痛程度、中医证候评分均低于治疗前（P＜0.05），且观察组低于对照组（P＜0.05）。两组大脑中动脉、大脑前动脉及大脑后动脉的Vs异常比率均低于治疗前（P＜0.05），且观察组低于对照组（P＜0.05）。观察组总有效率［86.7%（52/60）］高于对照组［68.3%（40/60）］（P＜0.05）。 结论天芎镇痛颗粒治疗肝风上扰型偏头痛可有效改善临床症状和脑血流动力学，临床效果明显。
Abstract:
ObjectiveTo observe clinical effects of Tianxiong analgesic granules in the treatment of migraine of liver wind harassing upward type. MethodsA total of 120 patients with migraine of liver wind harassing upper type were selected and randomly divided into an observation group and a control group using a random number table method, with 60 cases in each group. The control group was treated with flunarizine hydrochloride capsules, while the observation group was treated with Tianxiong analgesic granules. The course of treatment was 30 days for both groups. Comparisons were made between the two groups before and after treatment in terms of the frequency of headache attacks, duration of headache, degree of headache, TCM syndrome scores, the abnormal ratio of peak systolic blood flow velocity (Vs) in the middle cerebral artery, anterior cerebral artery, and posterior cerebral artery, as well as the clinical efficacy. ResultsAfter treatment, the frequency of headache attacks, duration of headache, degree of headache, and TCM syndrome scores in both groups were lower than those before treatment (P<0.05), and these parameters in the observation group were lower than those in the control group (P<0.05). The abnormal ratios of peak systolic blood flow velocity (Vs) in the middle cerebral artery, anterior cerebral artery, and posterior cerebral artery in both groups were lower than those before treatment (P<0.05), and the ratios in the observation group were lower than those in the control group (P<0.05). The total effective rate in the observation group [86.7% (52/60)] was significantly higher than that in the control group [68.3% (40/60)] (P<0.05). Conclusion Tianxiong analgesic granules can effectively improve clinical symptoms and cerebral hemodynamics in the treatment of migraine of liver wind harassing upper type, with significant clinical efficacy.
