摘要:
目的评估基于自我效能干预对妊娠期糖尿病（gestational diabetes mellitus，GDM）患者血糖及不良妊娠结局的影响。 方法计算机检索PubMed、Cochrane Library、Embase、Web of Science、EBSCO、中国期刊全文数据库、万方数据库、维普数据库和中国生物医学文献服务系统，搜集基于自我效能理论对妊娠期糖尿病患者进行干预的随机对照试验和类实验研究，由2位研究者独立筛选文献、提取资料和评价纳入研究的偏倚风险后，利用RevMan 5.3软件进行Meta分析。 结果纳入6项RCT研究，3项CCT研究，包括1113例患者。Meta分析结果显示：与常规护理相比，基于自我效能的干预能提高GDM患者自我效能（SMD=0.66，95%CI：0.08～1.24，P=0.02），改善患者空腹血糖（SMD=-1.04，95%CI：-1.51～-0.57，P=0.006）和餐后2 h血糖（MD=-1.69，95%CI：-2.64～-0.75，P=0.0005），减少不良妊娠结局发生率（RR=0.18，95%CI：0.11～0.30，P＜0.000 01）。 结论相比常规护理，基于自我效能的干预更能提高GDM患者自我效能，改善血糖水平和不良妊娠结局发生率；但受纳入研究数量和质量的限制，该结论尚需展开更多高质量研究予以验证。
Abstract:
ObjectiveTo evaluate the effects of self-efficacy-based interventions on glycemic control and adverse pregnancy outcomes in patients with gestational diabetes mellitus (GDM). MethodsA computerized search was conducted across the following databases: PubMed, Cochrane Library, Embase, Web of Science, EBSCO, China National Knowledge Infrastructure (CNKI), Wanfang Database, VIP Database, and Chinese Biomedical Literature Service System (SinoMed). Randomized controlled trials (RCTs) and quasi-experimental studies that employed self-efficacy theory-based interventions for patients with GDM were collected. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies. A meta-analysis was then performed using RevMan 5.3 software. ResultsSix RCTs and three controlled clinical trials (CCTs) were included, comprising a total of 1,113 patients. The results of the meta-analysis showed that, compared with conventional care, self-efficacy-based interventions significantly improved self-efficacy in GDM patients [SMD=0.66, 95% CI: 0.08 to 1.24, P = 0.02], reduced fasting blood glucose [SMD=-1.04, 95% CI: -1.51 to -0.57, P = 0.006] and 2-hour postprandial blood glucose [MD = -1.69, 95% CI: -2.64 to -0.75, P = 0.0005], and decreased the incidence of adverse pregnancy outcomes [RR = 0.18, 95% CI: 0.11 to 0.30, P<0.000 01]. ConclusionCompared with conventional care, self-efficacy-based interventions demonstrated superior effects in improving self-efficacy, glycemic control, and the incidence of adverse pregnancy outcomes in GDM patients. However, due to the limited number and methodological quality of the included studies, these conclusions should be corroborated by further high-quality investigations.
