从湿性年龄相关性黄斑变性（age related macular degeneration，AMD）起病隐匿、全身症状不显而难以辨证的临床难点入手，探讨舌诊作为中医特色诊法在该病辨证论治中的具体应用及理论依据，认为湿性AMD虽病在眼底黄斑，但其发生发展与全身脏腑、气血、津液失调密切相关，舌象能直观反映本病“本虚标实、虚实夹杂”的核心病机；通过追溯舌诊源流、结合现代医学发病机制及巢国俊教授的临床经验，认为临证当以舌象为主要辨证依据，将湿性AMD分为肝肾阴虚与脾虚痰湿（或夹热、夹瘀）两大证型进行论治，据此遣方用药可有效指导临床治疗。
Abstract:
Starting from the clinical difficulty of syndrome differentiation in wet age related macular degeneration (AMD)—characterized by insidious onset and lack of prominent systemic manifestations—this paper explores the specific application and theoretical basis of tongue diagnosis, a distinctive diagnostic method of traditional Chinese medicine (TCM), in the syndrome differentiation and treatment of this disease. It is proposed that although wet AMD is localized in the macular region of the fundus, its occurrence and development are closely related to systemic dysfunctions of the Zang-fu organs, Qi and blood, and body fluids. Tongue manifesta-tions can intuitively reflect the core pathogenesis of this disease, which is characterized by "root deficiency with branch excess" and "intermingled deficiency and excess." By tracing the historical origins of tongue diagnosis, integrating modern pathophysiological mechanisms, and drawing upon the clinical experience of Professor Chao Guojun, this paper proposes that tongue manifestations should serve as the primary basis for syndrome differentia-tion in clinical practice. Wet AMD can be classified into two main syndrome types: Liver-Kidney Yin Deficiency and Spleen Deficiency with Phlegm-Dampness (with possible concurrent Heat or Stasis). Prescriptions and medicinals selected accordingly can effectively guide clinical treatment.
