摘要:
目的：探讨和研究老年睡眠呼吸暂停低通气综合征(SAHS)患者的临床特点。方法：选取74例SAHS患者作为研究对象，通过多导睡眠图进行监测，分析其产生并发症情况的不同以及对不同年龄段的呼吸暂停、病情程度及经鼻持续气道正压通气的治疗效果。结果：74例患者中，存在并发症的患者占94.59%(70例)，合并两种或两种以上并发症者占52.70%(39例)。主要以高血压、冠心病、糖尿病以及脑血管疾病为主。60～69岁患者以阻塞性呼吸暂停为主；70岁以上患者以混合性呼吸暂停为主。70岁以上患者和60～69岁患者的平均呼吸暂停时间、平均SaO2以及最低SaO2有明显差异，具有统计学意义(P＜0.05)。结论：老年SAHS患者的并发症较多，而且随着年龄的增大，病情会随之发展，经鼻持续气道正压通气对SAHS具有较好的疗效。
Abstract:
Objective: To explore and study clinical characteristics of senile patients with sleep apnea hypopnea syndrome (SAHS). Method: Seventy-four patients with SAHS admitted to hospital in recent years were monitored with polysomnogram(PSG). The complications, apnea, severity and therapeutic effect of nasal continuous positive airway pressure ventilation treatment were analyzed. Result: Among these patients, 94.55%(70 cases) manifested complications, 52.70%(39 cases) demonstrated no less than two kinds of complications, mainly including hypertension, coronary heart disease, diabetes and cerebrovascular disease. Patients aged from 60 to 69 years mainly appeared obstructive apnea; the ones more than 70 years showed mixed apnea. Comparisons among the patients more than 70 years old and aged from 60 to 69 years had remarkable difference in average time of apnea, average SaO2 and the lowest SaO2 (P<0.05). Conclusion: Senile patients with SAHS would manifest more complications, the state of diseases get worse with age. Nasal continuous positive airway pressure ventilation has better curative effects.
