摘要:
目的：构建过敏性紫癜患儿的中西医结合临床护理路径，分析其在临床中的应用效果。方法：将过敏性紫癜患儿168例随机分为临床护理路径干预组(观察组)及常规护理干预组(对照组)，每组84例。观察并记录2组患儿的住院时间及费用；采用自制“健康教育达标情况表”对患儿家长的疾病知识、护理技能的掌握情况，以及遵医嘱行为进行评定；对护理满意度进行评价；采用PAQLQ评定生命质量。结果：与对照组比较，观察组患儿的住院时间及住院费用降低明显，疾病知识、护理技能掌握、遵医嘱行为的健康教育达标情况以及护理满意度升高明显，差异具有统计学意义(P＜0.05)。入院时，2组患儿的生命质量总分及各因子分(情感功能、活动受限以及临床症状)得分比较，差异无统计学意义(P＞0.05)；出院后6个月，与对照组比较，观察组患儿的生命质量总分及各因子分(情感功能、活动受限以及临床症状)得分均升高明显，差异具有统计学意义(P＜0.05)。结论：中西医结合临床护理路径可有效降低过敏性紫癜患儿的住院时间及费用，提高其家属疾病防治知识及技能，并能提高护理满意度，对于提升患儿的生命质量大有裨益。
Abstract:
Objective: To analyze the effects of integrative medicine clinical nursing pathway (CNP) in clinic by constructing integrative medicine CNP for the children with allergic purpura. Methods: All 168 children were randomized into the intervention group of CNP (the observation group) and routine nursing intervention group (the control group), 84 cases each group. Hospitalized time and expense of both groups were observed and recorded; the grasping conditions of disease knowledge and nursing skills of the parents and their behavior of following the doctor’s advice were investigated by selfmade “health education standard sheet”; nursing satisfaction degrees were assessed; and quality of life was assessed by PAQLQ. Results: Compared with the control group, hospitalized time and expense decreased notably, the grasping conditions of disease knowledge and nursing skills, health education conditions of following the doctor’s advice and nursing satisfaction degrees raised obviously in the observation group, the difference showed statistical meaning (P＜0.05). There was no statistical difference in the comparisons of total scales of quality of life and the scales of different factors (emotional function, limitation of motion and clinical symptoms) at admission time (P＞0.05); total scales of quality of life and the scales of different factors (emotional function, limitation of motion and clinical symptoms) raised notably when the observation group was compared with the control group in six months after discharged from the hospital, and the difference showed statistical meaning (P＜0.05). Conclusion: Integrative medicine CNP could effectively reduce hospitalized time and expense of the children with allergic purpura, improve the parents’ knowledge and skills for disease prevention and treatment and raise nursing satisfaction degrees, which are helpful to improve quality of life of the children.
