摘要:
目的：探讨中药熏洗对肛瘘术后患者的影响。方法：将肛瘘患者110例随机分为观察组与对照组各55例。两组患者术后采取常规抗感染处理，必要时肌肉注射镇痛药物。观察组加用中药熏洗方。比较两组患者创面面积、中医证候积分、表皮生长因子(epidermal growth factor，EGF)与直肠分泌物分泌型免疫球蛋白A(secreted immunoglobulin A，SIgA)变化情况。结果：治疗后两组患者创面面积均较治疗前减小，且观察组创面面积小于对照组(P＜0.05)。治疗后两组患者创面疼痛、渗液、肿胀及舌红苔黄、口苦舌燥、脉滑或弦的评分均较治疗前降低，且观察组低于对照组(P＜0.05)。治疗后两组患者EGF、SIgA均较治疗前升高，观察组高于对照组，差异有统计学意义(P＜0.05)。结论：在常规抗感染、镇痛的基础上应用中药熏洗能够有效促进肛瘘术后患者创面恢复，有助于调节EGF、SIgA水平。
Abstract:
Objective: To explore clinical effects of herbal fumigation on the recovery of the patients after the operation of anal fistula. Methods: All 110 patients were randomly divided into the observation group (n=55) and the control group(n=55). All the patients adopted conventional anti-infection treatment, muscular injection of analgesic drugs if necessary. The observation group accepted herbal fumigation prescription. To compare wound size, TCM syndrome integrals, EGF and SIgA between both groups. Results: After treating, wound areas of both groups reduced compared with before treating, and the wound area of the observation group was smaller than that of the control group (P＜0.05). After treating, the scores of wound pain, wound exudation, wound swelling, red tongue and yellow tongue coating, bitter mouth and dry tongue, slippery or taut pulse in both groups dropped compared with before treating, and the observation group was smaller than the control group(P＜0.05). After treating, EGF and SIgA of both groups rose compared with before treating, the observation group was higher than the control group, and the difference showed statistical meaning(P＜0.05). Conclusion: On the foundation of conventional anti-infection and analgesic treatment, herbal fumigation could effectively promote wound recovery after the operation of anal fistula, and it is helpful to regulate the levels of EGF and SIgA.
