摘要:
目的：观察益肾活血汤联合枸橼酸氯米芬胶囊治疗内分泌失调性不孕症临床疗效。方法：将468例患者随机均分为两组，每组234例。两组均给予枸橼酸氯米芬胶囊治疗，1粒/次，每日1次，月经后第5 天开始服用，连续5天；观察组同时服用益肾活血汤，每日1剂，早晚分服，服用3个月经周期。比较两组临床疗效、中医证候积分、性激素水平、卵泡直径、排卵率及1年妊娠率等指标。结果：总有效率对照组为78.63%(184/234)，观察组为87.18%(204/234)，对照组低于观察组，差异统计学意义(P＜0.05)。中医证候积分治疗前两组比较，差异无统计学意义(P＞0.05)；治疗前后两组组内比较，差异均有统计学意义(P＜0.05)；治疗后两组组间比较，差异也有统计学意义(P＜0.05)。血清卵泡刺激素(follicle stimulating hormone，FSH)、促黄体生成素(luteinizing hormone，LH)和雌二醇(estradiol，E2)治疗前两组比较，差异无统计学意义(P＞0.05)；治疗前后两组组内比较，差异均有统计学意义(P＜0.05)；治疗后两组组间比较，差异也有统计学意义(P＜0.05)。卵泡直径、排卵率及1年妊娠率观察组均高于对照组，两组比较，差异有统计学意义(P＜0.05)。结论：益肾活血汤联合克罗米芬可改善内分泌失调性不孕症患者的中医证候积分和血清性激素分泌水平，从而促进内分泌失调性不孕症患者卵泡发育并提升排卵率。
Abstract:
Objective: To observe clinical effects of kidney-nourishing blood-activating decoction and clomiphene citrate capsule in treating endocrine dysfunctional infertility. Methods: Altogether 468 patients were randomized into two groups, 234 cases each group. Both groups were given clomiphene citrate capsules, one capsule each time, once each day, they began to take the capsules at the fifth day since the period came, for five days consecutively; the observation group took kidney-nourishing blood-activating decoction, one dose each day, taking in the morning and evening, for three menstrual cycles. To compare clinical effects, TCM syndrome integrals, the levels of sex hormone, follicle diameter, ovulation rate and one year pregnancy rate and others between both groups. Results: Total effective rate of the control group was 78.63% (184/234), lower than 87.18% (204/234) of the observation group, and the difference showed statistical meaning (P＜0.05). The difference had no statistical meaning in TCM syndrome integrals between both groups before treating (P＞0.05),; the difference had statistical meaning before and after treating within two groups (P＜0.05); the difference showed statistical meaning between both groups after treating (P＜0.05). The difference had no statistical meaning in the levels of FSH, LH and E2 before treating between both groups (P＞0.05); the difference showed statistical meaning before and after treating within two groups (P＜0.05); the difference had statistical meaning between both groups after treating (P＜0.05). The observation group was higher than the control group in follicle diameter, ovulation rate and one-year pregnancy rate, the difference was statistically significant between both groups (P＜0.05). Conclusion: Kidney-nourishing blood-activating decoction and clomiphene citrate capsule could improve TCM syndrome integrals and the secretion levels of serum sex hormone of the patients suffering endocrine dysfunctional infertility, therefore to promote the follicular development and raise ovulation rate.
