摘要:
目的观察健脾益肾活血汤联合运动针灸治疗脾肾血瘀型排卵障碍性不孕症的效果。 方法对脾肾血瘀型排卵障碍性不孕症患者88例进行回顾性分析，根据治疗方法不同分为对照组40例和观察组48例。对照组予来曲唑及健脾益肾活血汤治疗，观察组在对照组治疗的基础上予运动、针灸治疗，观察两组临床疗效及预后情况。 结果总有效率观察组为97.9%（47/48），高于对照组的82.5%（33/40）（P＜0.05）。两组患者治疗后血清黄体生成激素(luteinizing hormone，LH)与睾酮(testosterone，T)水平均低于治疗前（P＜0.05），观察组低于对照组（P＜0.05）；血清抗苗勒氏管激素（anti-Müllerian hormone，AMH）水平低于治疗前，且观察组低于对照组（P＜0.05）；补体C1q肿瘤坏死因子相关蛋白［C1q/tumor necrosis factor(TNF)-related protein 12，CTRP12］水平高于治疗前，且观察组高于对照组（P＜0.05）。治疗后两组患者子宫动脉搏动指数（pulsatility index，PI）、阻力指数（resistance index，RI）均下降，且观察组低于对照组（P＜0.05）；治疗前后两组子宫动脉血流平均流速、收缩期流速比较差异无统计学意义（P＞0.05）。随访1年，观察组妊娠率为45.8%（22/48），高于对照组的12.5%（5/40）（P＜0.05）。 结论健脾益肾活血汤联合来曲唑及运动、针灸能降低脾肾血瘀型排卵障碍性不孕症患者AMH水平，提高CTRP12水平，调节机体性激素分泌平衡状态，从而提高治疗效果与妊娠率。
Abstract:
ObjectiveTo observe clinical effects of spleen-invigorating kidney-supplementing blood-activating decoction, exercise and acupuncture in the treatment of anovulatory infertility of blood stasis of spleen and kidney. MethodsAll 88 patients with anovulatory infertility of blood stasis of spleen and kidney were retrospectively analyzed, and allocated to 40 cases of the control group and 48 cases of the observation group according to therapeutic methods. The control group were treated by letrozole tablets and spleen-invigorating kidney-supplementing blood-activating decoction, the observation group were given exercise and acupuncture based on the therapy the control group accepted, to observe clinical effects and the prognosis of both groups. ResultsTotal effective rate of the observation group was 97.9%（47/48）, higher than 82.5%（33/40) of the control group (P<0.05). The levels of LH and T of both groups after treating were lower than these before treating (P<0.05), the observation group was lower than the control group (P<0.05). The levels of AMH were lower than these before treating, and the observation group was lower than the control group (P<0.05), the levels of CTRP12 were higher than these before treating, and the observation group was higher than the control group (P<0.05). After treating, PI and RI decreased in both groups, and the observation group was lower than the control group (P<0.05); the difference had no statistical meaning in average velocity and systolic velocity (P>0.05). After following up for one year, the pregnancy rate of the observation group was 45.8%(22/48), higher than 12.5%(5/40) of the control group (P<0.05). ConclusionSpleen-invigorating kidney-supplementing blood-activating decoction, letrozole tablets, exercise and acupuncture could lower the levels of AMH, raise the levels of CTRP12, regulate the balance of sex hormone secretion in the treatment of anovulatory infertility of blood stasis of spleen and kidney, therefore to improve therapeutic effects and pregnancy rate.
