基于文献分析放射性直肠炎中药灌肠液用药规律-《西部中医药》

摘要:: 目的搜集近十年治疗放射性直肠炎（radiation proctitis，RP）中药灌肠液方剂的文献，总结其组方用药，为放射性直肠炎的中医药治疗提供参考。方法以“放射性直肠炎”“灌肠”“灌肠液”“中药保留灌肠”“中药直肠滴注”为检索词，在中文科技期刊全文数据库（China science and technology journal database，VIP）、中国学术期刊全文数据库（China national knowledge infrastructure，CNKI）中进行搜索，根据既定标准纳入文献69篇，涉及药物132味，用药频次达618次。借助SPSS 19.0软件进行统计分析，研究药物的功效、性味、归经、升降浮沉。结果功效前3位的药物为清热药、补虚药、止血药，累积频率74.4%；寒性药为主，累积频率53.9%；苦、甘、辛味药累积频率78.0%；归经以肝、脾经为首，累积频率35.4%。结论清热解毒、补虚止血是放射性直肠炎的总治则，制备中药灌肠液药物推荐：黄连、甘草、地榆、黄柏、白及。

Abstract:: ObjectiveTo provide the reference for TCM therapy of radiation proctitis by collecting the literature on the prescriptions for herbal enema for RP in the recent ten years and summarize the prescriptions and medications. Methods“RP”, “enema”, “herbal retention enema” and “herbal rectal dripping” were chosen as the key words and searched in VIP and CNKI, according to the established criteria, 69 papers were included, involving 132 herbs and medication frequency reached 618 times. All the data were statistically analyzed via SPSS 19.0 software, to study the effects, the nature and property, entering meridians and the tendency of floating and sinking of the herbs. ResultsThe drugs with the top three efficacy were heat-clearing drug, tonic drugs and hemostatics, cumulative frequency was 74.4%; the cumulative frequency of the drug of cold nature was 53.9%; the cumulative frequency of the drug of bitter, sweet and pungent natures was 78.0%; the entering meridians were liver and spleen meridians, and cumulative frequency 35.4%. ConclusionHeat-clearing and detoxifying, tonifying the deficiency to stop bleeding is the general rule for RP, the drugs recommended for herbal enema are Huanglian (Coptis chinensis), Gancao (Glycyrrhiza uralensis Fisch.), Diyu (Garden Burnet Root), Huangbai (Cortex Phellodendri Chinensis) and Baiji [Bletilla striata (Thunb.) Rchb. f.].
