摘要:
目的观察中医理气化痰活血法联合胰岛素强化治疗与单纯胰岛素强化治疗诱导初发2型糖尿病患者“蜜月期”的疗效对比。 方法将120例初发2型糖尿病患者随机分为观察组和对照组各60例。对照组予单纯胰岛素泵强化治疗2周后改用门冬胰岛素（三餐前）+地特胰岛素（睡前）皮下注射；观察组在相同胰岛素治疗方案的基础上加服柴胡疏肝散合二陈汤，每日1剂，中药疗程为12周。对比两组患者的“蜜月期”诱导和持续缓解情况，治疗前后体质指数（body mass index，BMI）、空腹血糖（fasting blood glucose，FBG）、餐后2 h血糖（2-hour postprandial blood glucose，2h-PG）、糖化血红蛋白（glycosylated hemoglobin，HbA1c）、空腹C肽（fasting c-peptide，F-CP）、餐后2 hC肽（2-hour postprandial c-peptide，2h-CP）水平。 结果观察组成功诱导“蜜月期”率［21.6%（13/60）］高于对照组［8.3%（5/60）］（P＜0.05）；观察组缓解后失效率［7.7%（1/13）］与对照组［40%（2/5）］比较，差异无统计学意义（P＞0.05）。与治疗前比较，12周后两组患者的FBG、2h-PG、HbA1c、BMI水平均明显下降（P＜0.05），F-CP、2h-CP水平较本组治疗前明显上升（P＜0.05）；且观察组治疗后HbA1c水平和BMI指数较对照组同期下降明显（P＜0.05），观察组治疗后FBG、2h-PG、F-CP、2h-CP水平与对照组同期比较，差异无统计学意义（P＞0.05）。 结论中医理气化痰活血法联合胰岛素强化治疗能有效缓解初发2型糖尿病患者体内的高糖毒性，改善其机体失衡状态，促进2型糖尿病患者进入“蜜月期”。
Abstract:
ObjectiveTo compare clinical effects of combining the method of regulating Qi, resolving phlegm and activating blood circulation with insulin intensive treatment, and single insulin intensive treatment in inducing "honeymoon period" of newly diagnosed type two diabetes mellitus (T2DM). MethodsAll 120 T2DM patients were randomized into the observation group and the control group, with 60 cases in each group. The control group accepted single intensive insulin pump therapy for two weeks, switched to insulin aspart before three meals+subcutaneous injection of insulin detemir (before sleep); the observation group took Chaihu (Bupleuri Radix) soothing-liver powder and Erchen Tang on the foundation of the same insulin therapy, one dose each day, the course was 12 weeks. To compare the condition of inducing "honeymoon period" and sustainable mitigation condition, BMI, the levels of FBG, 2h PG, HbA1c, F-CP, 2h-CP before and after the treatment in both groups. ResultsThe rate of successfully inducing "honeymoon period" in the observation group was [21.6%(13/60)], higher than [8.3%(5/60)] of the control group (P<0.05); the difference had no statistical meaning in the failure rate after remission between the observation group [7.7%(1/13)] and the control group [40%(2/5)]. Compared with before treatment, the levels of FBG, 2h PG, HbA1c, and BMI were remarkably lowered in both groups after 12 weeks (P<0.05), the levels of F-CP and 2h-CP were raised compared with before the treatment apparently (P<0.05); the levels of HbA1c and BMI of the observation group after the treatment were lower than these of the control group remarkably in the corresponding period (P<0.05), the difference had no statistical meaning in the levels of FBG, 2h PG, F-CP and 2h-CP between the observation group and the control group after the treatment (P>0.05). ConclusionThe method of regulating Qi, resolving phlegm and activating blood circulation with insulin intensive treatment could effectively relieve the toxicity of high glucose in the patients with newly diagnosed T2DM, improve its imbalance in human body, and advance "honeymoon period" for T2DM patients.
