摘要:
目的探讨舒心通脉汤对病毒性心肌炎患者自噬相关通路的影响。 方法选择126例病毒性心肌炎患者，根据随机数字表法分为对照组和观察组，每组63例。对照组给予常规治疗，观察组在对照组基础上服用舒心通脉汤，疗程为4周。比较两组治疗前后外周血单核细胞（peripheral blood mononuclear cells，PBMCs）自噬相关基因Beclin-1、LC3-II mRNA及蛋白表达水平，凋亡相关基因Caspase-3、Bcl-2 mRNA水平，相关炎性细胞因子白细胞介素1β（interleukin-1β，IL-1β）、白细胞介素22（interleukin-22，IL-22）及肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）水平，比较两组临床疗效及不良反应发生情况。 结果与对照组比较，观察组治疗后外周血PBMCs中Beclin-1、LC3-II mRNA和蛋白表达水平明显降低，Caspase-3 mRNA明显降低，而抗凋亡基因Bcl-2 mRNA水平显著升高（P＜0.05）。观察组治疗后外周血IL-1β、IL-22和TNF-α水平显著低于对照组（P＜0.05）。观察组有效率［93.7%（59/63）］显著高于对照组［81.0%（51/63）］（P＜0.05）；治疗后两组不良反应发生率比较，差异无统计学意义（P＞0.05）。 结论舒心通脉汤治疗病毒性心肌炎患者有较好的临床疗效，其机制可能与降低细胞自噬及凋亡、减轻炎症反应有关。
Abstract:
ObjectiveTo explore the effects of Shuxin Tongmai decoction on autophagy related pathways in patients with viral myocarditis. MethodsAll 126 patients with viral myocarditis were divided into observation group and control group according to the random number table method, 63 cases in each group. The control group was given conventional treatment, and the observation group took Shuxin Tongmai decoction on the foundation of conventional therapy, the course of the treatment was four weeks. To compare the expressions of PBMCs autophagy - related genes Beclin-1, LC3-Ⅱ mRNA and protein, the levels of apoptosis-related genes Caspase-3 and Bcl-2 mRNA, the levels of related inflammatory factors including IL-1β, IL-22 and TNF-α between both groups before and after the treatment, to compare therapeutic effects and the incidence of adverse reaction between both groups. ResultsCompared with the control group, the levels of Beclin-1, LC3-Ⅱ mRNA and protein in PBMCs of peripheral blood of the observation group lowered clearly after the treatment, Caspase-3 mRNA decreased apparently while the levels of anti-apoptosis genes Bcl-2 mRNA increased remarkably (P<0.05). The observation group was lower than the control group in the levels of IL-1β, IL-22 and TNF-α after the treatment evidently (P<0.05). The effective rate of the observation group was [93.7%(59/63)], significantly higher than [81.0%(51/63)] of the control group (P<0.05); the difference had no statistical meaning in the incidence of adverse reaction between both groups after the treatment (P>0.05). ConclusionShuxin Tongmai decoction has better clinical effects in treating viral myocarditis, and its mechanism may be related to lowering cellular autophagy and apoptosis and reducing the inflammatory response.
