摘要:
目的探究棍点理筋法对腰椎间盘突出症患者疼痛及血清相关因子水平的影响。 方法选取120例腰椎间盘突出症患者作为研究对象，按照随机数字表法分为对照组和观察组，每组60例。对照组给予针灸联合放松推拿法，观察组给予针灸联合棍点理筋法，共治疗2个疗程。比较两组疼痛视觉模拟量表（visual analogue scale，VAS）评分、JOA腰痛（Japanese orthopaedic association，JOA）评分、Oswestry功能障碍指数（oswestry disability index，ODI）、临床疗效及血清前列腺素E2（prostaglandin E2，PGE2）、白细胞介素6（interleukin-6，IL-6）、降钙素基因相关肽（calcitonin generelated peptide，CGRP）、肿瘤坏死因子α（tumour necrosis factor-α，TNF-α）的差异。 结果治疗后VAS评分、ODI指数两组均较治疗前降低（P＜0.05），且ODI指数观察组低于对照组（P＜0.05）。治疗后JOA评分较治疗前改善（P＜0.05），且观察组高于对照组（P＜0.05）。观察组总有效率［93.33%（56/60）］显著优于对照组［75.00%（45/60）］（P＜0.05）。两组治疗2个疗程后血清PGE2、IL-6、CGRP、TNF-α水平均较治疗前降低（P＜0.05），且观察组治疗后上述指标均低于对照组（P＜0.05）。 结论棍点理筋法可显著减轻腰椎间盘突出症临床症状，提高临床疗效，可能与改善患者血清PGE2、IL-6、CGRP、TNF-α的水平相关。
Abstract:
ObjectiveTo investigate the effects of stick-point tendon-regulating method on pain and the levels of serum-related factors in patients with lumbar disc herniation (LDH). MethodsAll 120 LDH patients were chosen as the subjects investigated, and they were divided into the control group and the observation group in light of random number table method, 60 cases in each group. The control group was given acupuncture and relaxing massage method, and the observation group accepted acupuncture and stick-point tendon-regulating method, for two courses of the treatment. To compare VAS scores, JOA scores, ODI, clinical effects, the differen-ces in PGE2, IL-6, CGRP and TNF-α between both groups. ResultsAfter the treatment, VAS scores and ODI of the two groups were lower than these before the treatment (P<0.05), and the observation group was lower than the control group in ODI (P<0.05). After the treatment, JOA scores improved than before the treatment (P<0.05), and the observation group was higher than the control group (P<0.05). Total effective rate of the observation group was [93.33%(56/60)], significantly superior to [75.00%(45/60)] of the control group (P<0.05). The levels of PGE2, IL-6, CGRP and TNF-α were lower in the two groups after two courses of the treatment than these before the treatment (P<0.05), and the observation group was lower than the control group in the indexes above after the treatment (P<0.05). ConclusionStick-point tendon-regulating method could noticeably relieve clinical symptoms, raise clinical effects, possibly related to improving the levels of PGE2, IL-6, CGRP and TNF-α.
