摘要:
目的基于数据挖掘技术探究古籍医案方药对急性呼吸窘迫综合征的防治作用。 方法分别以“暴喘”“喘脱”“喘厥”为检索词，以“正文”为检索范围，检索并筛选中华医典（V5.0）电子软件收录的医论、医案类古籍中所涉及的相关方药，建立Excel方药数据库后，导入古今医案云平台（V2.3.5），对用药频次、中药四气五味归经属性进行统计，并运用聚类分析、复杂网络分析等方法探究古籍医案的用药规律及核心方药。 结果共纳入53首相关方剂，涉及中药189味，使用频次共计571次，以温、寒、平性及甘、苦、辛味药为主，归经多属肺、脾、胃、心、肾、肝经；使用5次及以上的中药35种，以陈皮、半夏、茯苓、紫苏子、苦杏仁、厚朴、白术、人参、泽泻为核心方药组成。 结论急性呼吸窘迫综合征作为新型冠状病毒感染、严重急性呼吸综合征等多种疾病危重期的常见并发症，古籍医案方药数据挖掘提示可从扶正、祛邪两方面早期辨证防治，祛邪方面重在化痰湿、清热毒、理气郁、逐血瘀，扶正方面则可益心肺、健中土、补肝肾、培气血，从而提高患者生存率，改善预后。
Abstract:
ObjectiveTo explore the preventive and curative effects of the prescriptions in ancient medical records for acute respiratory distress syndrome(ARDS) using data mining. Methods"Sudden dyspnea(暴喘)" "collapse syndrome caused by dyspnea(喘脱)" "syncope caused by dyspnea(喘厥)" were respectively chosen as the keywords, "text" as the retrieval range, to search and screen the related prescriptions involved in medical commentary, medical records collected in electronic software of Chinese medical dictionary (V5.0), after establishing the Excel database of prescriptions and herbs, the frequency of use, four Qi and five flavors, channel tropism and the nature of herbs were counted after importing the data into ancient and modern medical case cloud platform(V2.3.5), the medication rules and core prescriptions and herbs of ancient medical records were analyzed using cluster analysis and complex network analysis. ResultsA total of 53 relevant prescriptions were included, involving 189 herbs, the frequency of use reached 571 times, the herbs were mainly warm, cold and neutral in nature, sweet, bitter and pungent in flavor, entering the meridians of lung, spleen, stomach, heart, kidney and liver; 35 herbs were used more than five times, composed of Chenpi (Citri reticulatae pericarpium), Banxia(Pinelliae rhizoma), Fuling(Poria), Zisuzi(Perillae fructus), Kuxingren(Armeniacae semen amarum), Houpo(Magnoliae officinalis cortex), Baizhu(Atractylodis macrocephalae rhizoma), Renshen(Ginseng radix et rhizoma) and Zexie(Alismatis rhizoma) as the core prescription. ConclusionAs a common complication of COVID-19 and severe acute respiratory syndrome (SARS) at severe stage, ARDS could be prevented and treated in early differentiation by enhancing healthy Qi and eliminating the evils, suggested in data mining of the prescriptions in ancient medical records, thereby lifting the patients' survival rate and improving the prognosis, eliminating the evils means resolving phlegm-dampness, clearing heat toxin, regulating Qi depression and expelling blood stasis, and enhan-cing healthy Qi includes benefiting heart and lung, invigorating middle earth, tonifying kidney and liver and enri-ching Qi and blood.
