摘要:
目的观察自拟中药方治疗湿热型泌尿系结石的临床疗效及其对尿液成分、肾功能指标的影响。 方法选取泌尿系结石患者124例，按照随机数字表法分为观察组和对照组，每组62例。观察组采用自拟中药方治疗，对照组采用双氯芬酸缓释片口服治疗。比较两组治疗后临床疗效、中医证候积分，以及治疗前后尿液成分（尿比重）水平、肾功能指标［尿素、血清胱抑素C（cystatin C，Cys-C）、球蛋白（immunoglobulin，IgG）、白蛋白（albumin，Alb）］和生活质量评分。 结果观察组治疗后总有效率［88.71%（55/62）］明显高于对照组［64.52%（40/62）］（P＜0.05）。治疗后两组中医证候积分均较治疗前明显降低（P＜0.01），且观察组明显低于对照组（P＜0.01）。观察组治疗后尿比重明显低于对照组，差异有统计学意义（P＜0.01）。治疗后两组患者尿素、Cys-C明显低于治疗前，IgG、Alb水平明显高于治疗前（P＜0.05），上述指标两组比较，差异有统计学意义（P＜0.05）。治疗后观察组生活质量各项评分明显高于对照组，差异有统计学意义（P＜0.01）。 结论自拟中药方治疗下焦湿热型泌尿系结石临床效果显著，可明显改善患者临床症状及体征，提高患者肾功能及生活质量。
Abstract:
ObjectiveTo observe clinical effects of self-made Chinese medicine prescription in the treatment of urinary calculi of damp-heat in lower energizer pattern and its influence on urine ingredients and the indexes of renal function. MethodsAll 124 patients were chosen and divide into the observation group and the control group according to random number table method, with 62 cases in each group. The observation group was treated by self-made Chinese medicine prescription, and the control group took diclofenac sustained-release tablets orally. To compare clinical effects of both groups after the treatment, syndrome integrals, the levels of urine ingredients (urine specific gravity), the indexes of renal function [urea, Cys-C, IgG, Alb] and the scores of quality of life before and after the treatment. ResultsTotal effective rate of the observation group was [88.71%(55/62)] after the treatment, obviously higher than [64.52%(40/62)] of the control group (P<0.05). After the treatment, syndrome integrals of both groups lowered clearly than before the treatment (P<0.01), and the observation group was apparently lower than the control group (P<0.01). After the treatment, specific gravity of urine in the observation group was noticeably lower than that of the control group, and the difference had statistical meaning (P<0.01). After the treatment, urea and Cys-C of both groups were lower than these before the treatment, the levels of IgG and Alb were higher than these before the treatment (P<0.05), the difference was statistically significant in the comparisons of the indexes above (P<0.05). After the treatment, the scales of quality of life of the observation group were obviously higher than these of the control group, and the difference showed statistical meaning (P<0.01). ConclusionSelf-made Chinese medicine prescription could gain significant clinical effects in the treatment of urinary calculi of damp-heat in lower energizer pattern, it could improve clinical symptoms and body signs, body signs and raise the patients' renal function and quality of life.
