摘要:
目的系统评价青娥丸联合钙剂对绝经后骨质疏松症（postmenopausal osteoporosis，PMOP）的疗效是否优于单独使用钙剂。 方法检索CNKI、万方、VIP、Sinomed、PubMed、Cochrane Library、EMbase数据库，根据纳入与排除标准筛选文献并提取数据，采用Cochrane干预系统评价手册5.3版的偏倚风险评估工具进行质量评价；应用Review Manager 5.3软件进行Meta分析。 结果初步检索文献共90篇，最终纳入文献12篇。Meta分析结果显示：在增加临床有效率（OR=7.15，95%CI：2.26～22.61，P=0.000 8）、改善疼痛（SMD=-1.16，95%CI：-1.97～-0.75，P＜0.000 01）、增加腰椎骨密度（MD=0.02，95%CI：0.00～0.04，P=0.05）、改善血清I型胶原羧基端肽交联（β cross-linked C-telopeptideof type 1 collagen，β-CTX）（MD=-0.03，95%CI-0.05～-0.01，P=0.001）方面，青娥丸联合钙剂组明显优于常规钙剂组。在安全性方面：7项研究报告了服药后不良事件情况，主要有腹泻、便秘等胃肠道刺激症状及皮疹、肝功能异常等，停药后预后良好。 结论青娥丸联合钙剂在改善疼痛等症状、增加腰椎骨密度、改善血清β-CTX等生化指标，以及提高临床有效率方面具有一定优势。
Abstract:
ObjectiveTo systematically evaluate whether the clinical efficacy of Qing’e pills and calcium agents is superior to that of calcium agents in the treatment of postmenopausal osteoporosis (PMOP). MethodsBy retrieving CNKI, Wanfang, VIP, Sinomed, PubMed, Cochrane Library and EMbase databases, the literature was screened according to the inclusion and exclusion criteria for data extraction, quality evaluation was conducted using Cochrane risk of bias tool for intervention reviews from handbook version 5.3; Review Manager 5.3 was applied for Meta analysis. ResultsThe study primarily identified 90 papers, consequently included 12 papers. Meta analysis results displayed that the group of Qing’e pills and calcium agents was obviously better than the group of conventional calcium agents in increasing clinical effective rate (OR=7.15, 95%CI:2.26-22.61, P=0.0008), relieving pain (SMD=-1.16, 95%CI:-1.97-0.75, P<0.000 01) and adding lumbar bone density (MD=0.02, 95%CI:0.00-0.04, P=0.05), and improving serum β-CTX (MD=-0.03, 95%CI:-0.05--0.01, P=0.001). In terms of safety: seven studies reported adverse events after medication, the main symptoms include gastrointestinal irritation (e.g., diarrhea, and constipation), skin rashes and abnormal liver function, the prognosis is good after drug withdrawal. ConclusionThe combination of Qing’e pills and calcium agents has certain advantages in improving symptoms such as pain, adding lumbar bone density and improving serum β-CTX, and enhancing clinical effective rate.
