摘要:
目的：探讨危重症专职护理小组在慢性阻塞性肺疾病(慢阻肺)呼吸衰竭机械通气中的临床应用价值。方法：将80例患者随机分为观察组、对照组各40例，观察组行危重症护理小组干预，对照组实施常规护理，比较干预前后2组心、肺功能相关指标，统计住院时间、抢救成功例数及死亡率。结果：第一秒用力呼气容积(FEV1)、第一秒用力呼气容积/用力肺活量(FEV1/FVC)、左心室射血分数(LVEF)、肺动脉压(PAP)水平干预前后2组组内比较，差异有统计学意义(P＜0.05)，干预后2组组间比较，差异有统计学意义(P＜0.05)。住院时间2组比较，差异有统计学意义(P＜0.05)。观察组抢救成功36例(90.0%)，死亡4例(10.0%)；对照组抢救成功21例(52.5%)，死亡19例(47.5%)。抢救成功率、死亡率2组比较，差异有统计学意义(P＜0.05)。结论：针对慢阻肺呼吸衰竭使用呼吸机治疗患者实施危重症专职护理小组干预，可有效提高患者心肺功能，缩短住院时间，提高抢救成功率。
Abstract:
Objective: To discuss clinical application values of critical care group for COPD patients with respiratory failure who were treated by mechanical ventilation. Methods: Eighty patients were randomly allocated to the observation group and the control group, 40 cases each group, the observation group were intervened by critical care group and the control group accepted conventional care, mortality rate, the number of successful rescue and hospitalization time were counted, the related indexes of heart and lung functions of both groups were compared before and after the intervention. Results: The difference had statistical meaning in the comparisons of the levels of FEV1, FEV1/FVC, LVEF and PAP before and after the intervention within two groups (P＜0.05), the difference was statistically significant in the comparisons between both groups after the intervention (P＜0.05). The difference had statistical meaning in the comparisons of hospitalization time between both groups (P＜0.05). There were 36 cases resuced successfully (90.0%) and four cases died (10.0%) in the observation group, 21 cases resuced successfully (52.5%) and 19 cases died (47.5%) in the control group. The difference had statistical meaning in the comparisons of successful rescue rate and mortality rate between both groups (P＜0.05). Conclusion: The intervention of critical care group for COPD patients with respiratory failure who were treated by breathing machine could effectively raise heart and lung function of the patients, shorten hospitalization time and improve successful rescue rate.
