摘要:
目的对质子泵抑制剂（proton pump inhibitors，PPIs）联合半夏厚朴汤治疗胃食管反流病（gastroesophageal reflux disease，GERD）的疗效和安全性进行系统评价。 方法检索中国知网、万方、维普、中国生物医学文献服务系统、中国临床试验注册中心、Web of Science、Cochrane Library、PubMed、Embase，文种限制为中文及英文，收集自建库至2020年4月的PPIs联合半夏厚朴汤治疗GERD的临床随机对照试验（RCTs）。根据纳入和排除标准筛选文献，提取资料后采用RevMan5.3软件进行疗效和安全性的Meta分析。 结果共纳入14篇文献，Meta分析结果显示：在临床疗效（P＜0.00001，RR=1.23，95%CI［1，17，1.29］）、内镜下有效率（P＜0.00001，RR=1.21，95%CI［1，12，1.31］）、不良反应发生率（P=0.03，RR=0.38，95%CI［0.15，0.92］）、临床症状RDQ评分（WMD=-0.96，95%CI［-1.12，-0.80］，P＜0.00001）、GerdQ评分（WMD=-2.17，95%CI［-2.79，-1.56］，P＜0.00001）、食管括约肌静息压（WMD=3.57，95%CI［1，15，6.00］，P=0.004）方面试验组均优于对照组，差异有统计学意义。 结论PPIs联合半夏厚朴汤治疗GERD的疗效及安全性较好，优于单纯PPIs等西药治疗，在改善患者症状、食管黏膜炎症方面具有一定优势。
Abstract:
ObjectiveTo systematically assess clinical efficacy and the safety of PPIs and Banxia Houpo Tang in the treatment of gastroesophageal reflux disease (GERD). MethodsRCTs on PPIs and Banxia Houpo Tang in treating GERD were retrieved from CNKI, Wanfang, VIP, SinoMed, Chinese clinical trial registry, Web of Science, Cochrane Library, PubMed and Embase, the language limit is Chinese and English, between database building time and April, 2020. The papers were screened according to the inclusion and exclusion criteria, after extracting the data, RevMan 5.3 software was used to conduct Meta-analysis of clinical efficacy and the safety. ResultsAltogether 14 papers were included, Meta analysis showed that clinical efficacy (P<0.00001, RR=1.23, 95%CI[1.17, 1.29]), endoscopic effective rate (P<0.00001, RR=1.21, 95%CI[1.12, 1.31]), the incidence of adverse reaction (P=0.03, RR=0.38, 95%CI[0.15, 0.92]), RDQ scores of clinical symptoms (WMD=-0.96, 95%CI[-1.12, -0.80], P<0.00001), GerdQ scores (WMD=-2.17, 95%CI[-2.79, -1.56], P<0.00001), and the resting pressure of esophageal sphincter (WMD=3.57, 95%CI[1.15, 6.00], P=0.004), the trial group was superior to the control group, and the difference had statistical meaning. ConclusionPPIs and Banxia Houpo Tang show better clinical efficacy and safety in the treatment of GERD, superior to these of Western medicine therapy of single PPIs, and it possesses certain advantages in improving the patients' symptoms and esophageal mucosal inflammation.
