摘要:
目的：利用系统生物学技术初步探索糖尿病视网膜病变(DR)阳虚病机的代谢组学基础，以阐释阳虚病机主导DR病情进展的关系。方法：基于前期临床研究结果拟定DR阳虚证候调查表，根据阳虚证的有无，采集89例2型DR患者，并依据中医症状分为无阳虚证组和有阳虚证组，统一收集所有观察对象血浆样本，应用气相色谱-飞行时间质谱(GC-TOFMS)技术进行血浆全代谢组学检测，建立DR分期、阳虚证的血浆代谢组指纹图谱，运用OPLS化学计量学统计方法处理各图谱信息，进行组间代谢组学差异分析，建立相应代谢指纹图谱数据库。结果：代谢组学差异分析显示DR有阳虚证组和无阳虚证组基本分离，较少重叠，临床前期和非增殖期基本重叠，而增殖期和非增殖期基本分离，重叠较少；DR有阳虚证与无阳虚证、增殖期与非增殖期之间在代谢组学方面存在差异。结论：阳虚可能影响机体能量代谢致使机体内环境发生重大变化，诱导引起DR增殖性改变的新生血管因子相关基因表达，使DR从非增殖期转化为增殖期，说明阳虚为DR病情发展的关键病机。
Abstract:
Objective: To explore the metabonomics of Yang deficiency mechanism with systematic biology in order to illustrate the relationship between Yang deficiency and the progression of DR. Method: Yang deficiency syndromes questionnaires of DR were designed according to previous clinical study. Eighty-nine patients with diabetic retinopathy type two were divided to the group of Yang deficiency and without Yang deficiency, serum specimens of all the patients were collected and the metabonomics of the serum were detected with GC-TOFMS technology, the phases of DR and plasma metabolism fingerprint of Yang deficiency pattern were established, the information of different fingerprints was processed with OPLS to analyze the difference between groups in metabonomics and establish the corresponding database. Result: The differences of metabonoics outcomes revealed that the groups of Yang deficiency and without Yang deficiency were basically separated with few overlap, previous and non-hyperplasia phase were overlap, but proliferation and non-proliferation phases were basically separated with few overlap; the differences were existed between the groups of Yang deficiency and without Yang deficiency, the proliferation and non-proliferation phases. Conclusion: Yang deficiency might affect the energy metabolism and the great changes of internal environment, which could induce the gene expressions of new vessel factor, the gene could induce DR changing from non-proliferation phase to proliferation period, it shows that Yang deficiency is the key factor of DR progression.
