摘要:
目的：分析类风湿关节炎(RA)患者不同中医证型的临床特点及实验室指标，为临床诊疗提供参考依据。方法：回顾性分析2013年7月至2014年6月在新疆医科大学附属中医医院风湿科住院的223例RA患者，记录其临床特点及实验室指标，并进行统计分析。结果：痰瘀痹阻证RA患者中男性居多，各中医证型间男女比例差异有统计学意义(P＜0.01)；湿热痹阻型RA患者血小板水平高于其他3种证型，差异有统计学意义(P＜0.01)；湿热痹阻型RA患者血中纤维蛋白原水平高于其他3种证型，与肝肾阴虚型比较差异有统计学意义(P＜0.05)；各中医证型患者的DAS28评分、血沉、C反应蛋白及类风湿因子、抗角蛋白抗体、抗环瓜氨酸肽抗体阳性率比较差异无统计学意义(P＞0.05)。结论：湿热痹阻型RA疾病活动度高，病情较重。
Abstract:
Objective: To provide the reference for clinical diagnosis and treatment by analyzing lab indicators and clinical features of different patterns of the patients with rheumatoid arthritis (RA). Methods: The data of 223 RA patients who were hospitalized in TCM Hospital Affiliated to Xinjiang Medical University from July, 2013 to June, 2014 were retrospectively analyzed, their clinical features and lab indicators were recorded and statistically analyzed. Results: Most of the patients were male among RA patients of phlegm and blood stasis obstruction pattern, and the difference had statistical meaning in the comparisons of the ratio of female and male among different patterns (P＜0.01); the levels of blood platelet of RA patients of dampness-heat obstruction pattern were obviously higher than these of RA patients of other three patterns, and the difference showed statistical meaning (P＜0.01); fibrinogen level of RA patients of dampness-heat obstruction pattern was higher than that of RA patients with other three patterns, and the difference showed statistical meaning compared with the patients of liver-kidney Yin deficiency pattern(P＜0.05); the difference had no statistical meaning in the comparison of DAS28 scales, ESR and CRP, of rheumatoid factors (RF), antikeratin antibody (AKA) and the positive rate of anti-cyclic citrulinated peptide antibodies (ACCP) among the patients of different patterns (P＞0.05); the difference had no statistical meaning in the comparisons among the patients of different patterns (P＞0.05). Conclusion: RA patients of dampness-heat obstruction pattern show high activity of the disease and severe disease condition.
