摘要:
目的：从影像学角度探讨腰椎间盘突出症中医辨证分型与西医病理的相关性。方法：选取150例腰椎间盘突出症患者进行中医辨证分型，对所有患者行CT、MRI扫描并记录影像数据，对结果进行统计分析。结果：气滞血瘀型、风寒湿滞型以旁中央型突出为主；湿热痰滞型以中央型突出为主；肝肾亏虚型以中央型突出为主。气滞血瘀型腰椎间盘突出症患者，多见腰椎骨质增生，椎间孔的狭窄，而且常出现曲度变直现象；湿热痰滞型腰椎间盘突出症患者多见骨质增生样变和生理曲度的变化；风寒湿滞型腰椎间盘突出症患者多见腰椎管狭窄；肝肾亏虚型腰椎间盘突出症患者并发症较为复杂，腰椎的曲度变化、腰椎管的狭窄及骨质增生都多见。结论：从影像学角度分析腰椎间盘突出症的中医证型与西医病理具有密切的相关性。
Abstract:
Objective: To explore the relationship between TCM syndrome types and pathology of Western medicine of Lumbar disc herniation(LDH). Methods: 150 patients with LDH in our hospital were selected, and TCM syndrome type differentiations were carried out. At the same time, all patients were scanned with CT and MRI, and the imaging data were recorded as well. In addition, the results were analyzed statistically. Results: The paramedian lumbar disc herniation was common in patients with Qi stagnation and blood stasis and obstruction of wind, cold and dampness while the central lumbar disc herniation was common in patients with obstruction of dampness, heat and phlegm and deficiency of liver and kidney. For patients with lumbar disc herniation in types of Qi stagnation and blood stasis, most of them suffered from hyperosteogeny of the lumber vertebra, intervertebral space stenosis, and vertebral physiological curvatures became straight. For patients with lumbar disc herniation in types of obstruction of dampness, heat and phlegm, most of them came with hyperosteogeny lesion and changes of vertebral physiological curvatures. Besides, for patients with lumbar disc herniation in types of obstruction of wind, cold and dampness, they appeared with lumbar spinal canal stenosis. While for patients with types of deficiency of liver and kidney, complications were complicated. It was common of them suffered with changes of vertebral physiological curvatures, lumbar spinal canal stenosis and hyperosteogeny. Conclusion: From the perspective of imaging, there is a close correlation between TCM syndrome types of lumbar disc herniation and pathology of Western medicine.
