摘要:
目的：观察当归四逆汤联合鼠神经生长因子治疗糖尿病周围神经病变(DPN)的临床疗效。方法：将DPN患者40例随机分为治疗组、对照组各20例，治疗组予当归四逆汤和鼠神经生长因子治疗，对照组予前列地尔及鼠神经生长因子，均治疗4周。记录治疗前后2组患者腓总神经运动神经传导速度(MNCV))和腓肠神经感觉神经传导速度(SNCV)、多伦多临床评分系统(TCSS)评分、临床疗效及不良反应。结果：1)治疗后2组患者MNCV、SNCV均较治疗前提高，差异有统计学意义(P＜0.05)；2组间MNCV及SNCV比较，差异无统计学意义(P＞0.05)；2)2组TCSS评分均较治疗前降低，治疗组优于对照组，差异有统计学意义(P＜0.05)；3)治疗组总有效率为95.0%，高于对照组的60.0%，差异有统计学意义(P＜0.05)；4)常见不良反应治疗组为消化道不适，均可耐受，对照组为静脉炎，需改变用药方式，1例出现不可耐受。结论：当归四逆汤联合鼠神经生长因子以及前列地尔联合鼠神经生长因子均能提高DPN的神经传导速度，但前者能更好地改善患者临床症状及体征，且不良反应较少，疗效更佳。
Abstract:
Objective: To observe clinical effects of Danggui Sini Tang combined with rat nerve growth factor in the treatment for diabetic peripheral neuropathy (DPN). Methods: Forty patients were randomized into the treatment group and the control group, the treatment group was treated by Danggui Sini Tang combined with rat nerve growth factor, while the control group was given alprostadil combined with rat nerve growth factor, the course was four weeks. To record adverse reaction, clinical effects, Toronto clinical scoring system (TCSS) scores, the sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity (MNCV) before and after treating between both groups. Results: 1) MNCV and SNCV of both groups raised after treating compared with before treating, and the difference showed statistical meaning (P＜0.05); the difference had no statistical meaning in the comparisons of MNCV and SNCV between both groups(P＞0.05); 2) TCSS scores of both groups reduced than before treating, the treatment group was superior to the control group, and the difference had statistical meaning (P＜0.05); 3) Total effective rate of clinical effects in the treatment group was 95.0%, higher than 60.0% of the control group, and the difference had statistical meaning (P＜0.05); 4) Common adverse reaction in the treatment group was digestive tract discomfort, and the patients could tolerant the discomfort, while common adverse reaction of the control group was phlebitis, and the medication style needs to be changed, one patient was intolerant. Conclusion: Danggui Sini Tang combined with rat nerve growth factor, and alprostadil combined with rat nerve growth factor could raise nerve conduction velocity in the treatment for DPN, the former could improve clinical synptoms and body signs with fewer reactions and best therapeutic effects.
